痔术后出血原因分析及对策

摘　　要：为分析痔术后出血的原因,探讨防治出血的对策,选取本院收治的44例痔术后出血患者作为研究对象,统计出血发生率、出血时间、出血量,分析术后出血的原因,探讨术后出血与手术方式、出血量与出血时间的关系。结果显示,吻合器痔上黏膜环切术(PPH)后出血的发生率为4.4%,显著高于痔切除术(P <0.01),痔术后大量出血病例中88.9%为PPH术后出血,少量出血多发生于术后早期,大量出血可发生于术后早、中、晚期。结果表明,痔术后出血与手术方式密切相关,不宜盲目扩大PPH手术指征,痔术后出血早发现、早治疗非常重要。This study was to analyse the causes of postoperative bleeding of hemorrhoids,and search for the relevant target measures,enrolled 44 cases met above condition treated in authors’ hospital as research objects:statistically record bleeding incidence,time and volume,analyse the causes,and search for the relation of bleeding to procedure,and for the relation of volume to time.As results,the incidence from PPH was significantly higher than that from hemorrhoidectomy(4.4% vs 1.9%,P <0.05);in the cases with massive bleeding,the cases with bleeding after PPH were accounted for 88.9%;less bleeding more happened at early stage after surgery,while massive bleeding just at every stages(early,mid-,and late)after surgery.Results show that postoperative bleeding of hemorrhoids is closely related to procedures,therefore,it should not be blind enlarge surgical indication of PPH,and,it is very important to early discovery and treatment.
