藻酸盐银敷料联合光子治疗仪用于小儿肛周脓肿术后的效果观察

摘　　要：为探究小儿肛周脓肿术后采用藻酸盐银敷料换药联合光子治疗仪局部照射的治疗效果,选取160例小儿肛周脓肿患儿,随机分为观察组和对照组,各80例。观察组采用藻酸盐银敷料换药联合光子治疗仪局部照射治疗,对照组单纯采用藻酸盐银敷料换药治疗。比较2组创面愈合时间、肛周感染控制时间、肛门功能及肛门直肠压力等。结果显示,观察组创面愈合时间、肛周感染控制时间、肛门功能恢复时间均短于对照组(P <0.05),换药总次数少于对照组(P <0.05)。治疗20d,观察组直肠静息压、肛管静息压小于对照组;肛管最大收缩压、括约肌最大耐受阈值、括约肌最大收缩压均大于对照组(P <0.05)。观察组便秘评分小于对照组,舒适感评分高于对照组(P <0.05)。2组并发症及后遗肛瘘发生率比较,差异无统计学意义(P>0.05)。结果表明,小儿肛周脓肿术后采用藻酸盐银敷料换药结合光子治疗仪局部照射效果显著,可促进创口愈合,改善肛门功能及预后。This study was to observe the efficacy of alginate silver dressing(ASD)combined with photon therapeutic apparatus(PTA)applied after perianal abscess(PAA)surgery,randomly divided 160 cases of infantile PAA into observation group(80 cases,received ASD for dressing exchange and PTA for local radiation)and control group(80 cases,only ASD for dressing exchange);then,compared both groups’ wound-surface healing time,time for perianal infection controlled,anal function and ano-rectal pressure,etc.As results,in the time for wound-surface healing,for perianal infection controlled and for anal function recovered;in total time-numbers of dressing exchange;in rectal resting pressure and anal resting pressure20 days after treatment;in anal max.systolic pressure,sphincter’s max.tolerance threshold,and sphincter’s max.systolic pressure;in constipation ratings;and,in comfortable sense ratings,observation group was respectively shorter,less,smaller,larger,smaller,and higher than control group(all,P <0.05).Only in the incidence of complication,and sequel anal fistula there was no statistical difference between the two groups(P >0.05).Results show that for infantile PAA,ASD for dressing exchange and PTA for local radiation can achieve significant efficacy,promote wound healing,as well as improve anal function and prognosis.
