常规超声对于儿童输尿管开口异位的诊断价值

摘　　要：目的探讨常规超声对儿童输尿管开口异位的诊断价值。方法收集经手术证实的输尿管开口异位病例70例,回顾性分析其术前超声表现,以术中所见及病理为准.记录引流的肾脏表现。常规超声观察肾脏大小、形态、回声、肾盂及输尿管,观察膀胱三角区有无输尿管开口,评估常规超声对有否输尿管开口异位的诊断效能,并分别比较常规超声与静脉肾盂造影(IVP)、逆行尿路造影(VCUG)的诊断效能差异。针对存在输尿管开口异位的重复肾,评估常规超声提示完全性和不完全重复肾的诊断符合率。结果常规超声诊断有否输尿管开口异位的符合率为77.2%,优于IVP、VCUG(P=0.000,0.000)。在所有合并输尿管开口的重复肾中,超声正确甄别完全重复肾及不完全重复肾的符合率为100%。结论常规超声诊断有否输尿管开口异位具有较高符合率,因此可以作为输尿管开口异位的重要检查方法。Objective To explore the diagnostic value of conventional ultrasound in children with ectopic ureter.Methods 70 cases of ectopic ureteral orifice were collected,and the preoperative ultrasound findings were analyzed retrospectively.Based on the intraoperative findings and pathology,the manifestations of the relative kidney were recorded.Using conventional ultrasound to observe the kidney size,shape,echo,renal pelvis and ureter,and to observe the presence of ureteral opening in the trigone of the bladder.We evaluated the diagnostic efficacy of conventional ul-trasound for ectopic ureter.The diagnostic efficiency of conventional ultrasound,intravenous pyelography(I VP)and voiding cystourethrogram(VCUG)were compared to each other.For duplicate kidneys with ectopic ureter,the diagnostic coincidence rate of complete and incomplete duplicate kidneys diagnosed by conventional ultrasound was evaluated.Results The coincidence rate of conventional ultrasound diagnosis of ectopic ureter was 77.2%,which was better than IVU.RU.(P=0.000,0.000).In all duplication kidneys with ectopic ureter,the coincidence rate of ultrasound in distinguishing complete duplication kidney or not was 100%.Conclusions Diagnosis of ectopic ureter by conven-tional ultrasound has a high coincidence rate.Therefore,it can be used as a important examination for ectopic ureter.
