摘　　要：目的:分析经食道超声(TEE)引导下经胸小切口封堵术治疗中央型房间隔缺损的疗效。方法:2009年11月-2019年11月收治中央型房间隔缺损患者50例,随机分为两组,各25例。参照组应用常规经皮导管封堵术;研究组应用TEE引导下经胸小切口封堵术。比较两组治疗效果。结果:研究组手术时长、术中出血量、卧床时长、住院时长均明显低于参照组,差异有统计学意义(P<0.05);研究组治疗总有效率明显高于参照组,差异有统计学意义(P<0.05);研究组术后3个月左心室收缩末期内径(LVESD)及左室舒张末期内径(LVEDD)水平均低于参照组,左室射血分数(LVEF)水平高于参照组,差异均有统计学意义(P<0.05)。结论:TEE引导下经胸小切口封堵术治疗中央型房间隔缺损的疗效显著,可有效改善患者预后和心功能,促进患者快速恢复。Objective:To explore the efficacy of TEE guided transthoracic small incision closure in the treatment of central atrial septal defect.Methods:From November 2009 to November 2019,50 patients with central atrial septal defect were selected.They were randomly divided into two groups with 25 cases in each group.The reference group was treated with conventional percutaneous catheter occlusion.The study group used TEE guided transthoracic small incision closure.We compared the therapeutic effects of the two groups.Results:In the study group,the length of operation,the amount of intraoperative bleeding,the length of stay in bed and the length of hospitalization were significantly lower than those in the reference group,the difference was statistically significant(P<0.05).The total effective rate of the study group was significantly higher than that of the reference group,the difference was statistically significant(P<0.05).In the study group,the levels of left ventricular end systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDd)3 months after operation were lower than those in the reference group,and the level of left ventricular ejection fraction(LVEF)was higher than those in the reference group,the difference was statistically significant(P<0.05).Conclusion:The efficacy of TEE guided transthoracic small incision closure in the treatment of central atrial septal defect is significant,it can effectively improve the prognosis and cardiac function of patients and promote the rapid recovery of patients.
