新型冠状病毒肺炎患者胸部CT表现与临床分型的相关性研究

摘　　要：目的:探讨新型冠状病毒肺炎(NCP)患者胸部CT影像表现与临床分型的关系。方法:2020年1-3月收治NCP患者45例,回顾性分析临床资料和胸部CT影像学资料,将左右肺均为10等分,按每个肺段占5%为基准,统计分析累及严重程度与临床分型的关系。结果:45例NCP患者中,轻型18例,普通型20例,重型6例,危重型1例,无死亡病例。NCP患者胸部CT主要表现形态为斑片状/片状/团片状磨玻璃影、斑片状/弥漫性实变影、网格影或条索影。病变累及范围与临床分型存在相关性,差异有统计学意义(P<0.001)。结论:NCP具有临床表现的多样性,各临床分型存在相对差异性。本研究发现,通过患者胸部CT的表现、病灶累及的肺段初步评估临床分型,能对其临床病程及严重程度进行评估。Objective:To explore the relationship between the chest CT imaging and clinical classification of patients with new coronavirus pneumonia(NCP).Methods:From January to March 2020,45 patients with NCP were selected.The clinical data and chest CT imaging data were retrospectively analyzed.The left and right lungs were divided into 10 equal parts,and each lung segment accounted for 5%.The relationship between the severity of involvement and clinical classification was statistically analyzed.Results:Of the 45 NCP patients,18 were mild,20 were normal,6 were severe,and 1 was critical.There were no deaths.The main manifestations of chest CT in patients with NCP are patchy/slice/bulk ground glass shadows,patchy/diffuse consolidation shadows,grid shadows or striped shadows.The extent of lesion involvement was correlated with clinical classification,the difference was statistically significant(P<0.001).Conclusion:NCP has a variety of clinical manifestations,and there are relative differences in various clinical types.This study found that the clinical course and severity of the patient's clinical course and severity can be assessed through the preliminary assessment of the clinical classification of the patient's chest CT findings and the lung segment involved.
