摘　　要：目的:探讨心电图QRS波时限与慢性心力衰竭(CHF)患者主要心脏不良事件发生的关系。方法:选取2018年1月至2021年1月宜春市第二人民医院78例CHF患者为研究对象,性别不限,年龄52~76岁。所有患者均接受心电图检查,随访6个月,统计心脏不良事件发生情况,分析心电图QRS波时限与CHF患者主要心脏不良事件发生的关系。结果:随访6个月,78例CHF患者中发生心脏不良事件25例(32.1%),未发生心脏不良事件53例(67.9%);发生组心功能、左室射血分数、左室舒张末期内径、QRS波时限与未发生组比较,差异有统计学意义(P<0.05)。Spearman相关分析显示,心电图QRS波时限与CHF患者发生心脏不良事件呈正相关(r=0.319,P<0.001)。心电图QRS波时限评估CHF患者发生心脏不良事件的曲线下面积(AUC)>0.8。结论:心电图QRS波时限在CHF患者中具有良好的评估价值,心电图QRS波时限越长CHF患者心脏不良事件的发生率越高。Objective: To explore the relationship between QRS duration and major adverse cardiac events in patients with chronic heart failure(CHF). Methods: Seventy-eight patients with CHF in Yichun Second People’s Hospital from January 2018 to January 2021 were selected as the research objects, both of sexes, aged 52-76 years. All patients underwent ECG examination and were followed up for 6 months. The incidence of adverse cardiac events was counted, and the relationship between QRS duration and the occurrence of major adverse cardiac events in patients with CHF was analyzed. Results: After 6 months follow-up, among 78 patients with CHF, 25 cases(32.1%) had adverse cardiac events(occurrence group) and 53 cases(67.9%) had no adverse cardiac events(non-occurrence group). There were significant differences in cardiac function, left ventricular ejection fraction,left ventricular end diastolic diameter and QRS duration between the two groups(P<0.05). Spearman correlation analysis showed that QRS duration was positively correlated with adverse cardiac events in patients with CHF(r=0.319, P<0.001). The area under curve(AUC) for evaluating adverse cardiac events in patients with CHF by QRS duration was > 0.8. Conclusion: The QRS duration has a good evaluation value in patients with CHF. The longer the QRS duration, the higher the incidence of adverse cardiac events.
