摘 要:目的研究连续肾脏替代(CRRT)联合液体复苏治疗脓毒血症致急性肾损伤(AKI)疗效观察及对血清TNF-α,IL-6及免疫功能的影响。方法抽取本院于2017年12月至2020年12月收治的脓毒血症致AKI患者共92例,参照随机抽样法分成对照组(46例,实施单纯液体复苏治疗方式)和观察组(46例,实施CRRT联合液体复苏治疗方式)。结果与对照组对比,观察组脓毒血症致AKI患者治疗后的血清TNF-α和IL-6指标更低(P<0.05)。与对照组对比,观察组脓毒血症致AKI患者的有效率更高(P<0.05)。观察组脓毒血症致AKI患者的CD4^(+)、CD3^(+)与CD4^(+)/CD8^(+)指标远高于对照组(P<0.05)。结论CRRT结合液体复苏治疗对于改善患者的炎症反应,提高机体免疫功能等有重要意义。Objective To study the clinical effect of continuous renal replacement(CRRT)combined with fluid resuscitation in treatment of sepsis induced acute kidney injury(AKI)and its influence on serum TNF-α,IL-6,and immune function.Methods 92 cases of patients with AKI caused by sepsis and who treated in our hospital from December 2017 to December 2020 were selected,and the patients were randomly divided into control group(46 cases,treated with liquid resuscitation alone)and observation group(46 cases,treated with CRRT combined with liquid resuscitation).Results The serum TNF-αAnd IL-6 after treatment in the observation group of AKI patients with sepsis was significantly higher than those in the control group(P<0.05).The effective rate of AKI patients caused by sepsis in the observation group was higher than that in the control group(P<0.05).The serum TNF levels in the observation group was significantly higher than those in the control groupαAnd IL-6 were lower(P<0.05).Conclusion The CRRT combined with liquid resuscitation is great significance to improve the inflammatory response and immune function of patients.
