摘　　要：目的分析青年女性人工流产术后即刻放置吉妮致美宫内节育器的临床效果和不良反应。方法选择2019年1—12月在玉环市中医院行人工流产的青年女性169例,其中89例人工流产术后即刻放置吉妮致美宫内节育器的患者为观察组1,80例人工流产术后不放置宫内节育器,也未口服避孕药的患者为对照组。选择同期在玉环市中医院月经后3~7 d放置吉妮致美宫内节育器的100例青年女性为观察组2。比较临床效果和不良反应。结果观察组1和观察组2放置吉妮致美宫内节育器1、3、6、12个月带器妊娠率、脱落率、取出率、异常出血率比较差异均无统计学意义(均P>0.05)。3组总出血天数、滴血天数、平均每次出血和(或)滴血天数比较差异均有统计学意义(均P<0.05)。随访12个月,观察组1和观察组2不良反应发生率比较差异均无统计学意义(均P>0.05)。结论青年女性人工流产后即刻与经期后放置吉妮致美宫内节育器有同等的避孕效果,两者成功率均较高、操作简单,不良反应少,但即刻放置可避免宫腔操作的二次痛苦。Objective To analyze the clinical effect and adverse reactions of immediate application of Jenefox intrauterine device after induced abortion in young women.Methods From January to December in 2019, 169 young women undergoing induced abortion in Yuhuan Municipal Hospital of Traditional Chinese Medicine were selected and divided into observation one group(89 cases, immediate application of Jenefox intrauterine device after induced abortion) and control group(80 cases, induced abortion, without intrauterine device and oral contraceptive).One hundred young women treated with Jenefox intrauterine device at 3-7 days after menstruation during the same period in Yuhuan Municipal Hospital of Traditional Chinese Medicine were selected as observation two group.The clinical effects and adverse reactions were compared.Results There was no statistically significant difference in pregnancy rates with IUD, IUD expulsion rates, IUD removal rates, and abnormal bleeding rates at 1, 3, 6, and 12 months after IUD application between observation one group and observation two group(P>0.05).There were statistically significant differences in the total time of bleeding, bleeding time, the bleeding time every time among the three groups(P<0.05).The young women were followed up for 12 months, there was no statistically significant difference in the incidence rate of adverse reactions between observation one group and observation two group(P>0.05).Conclusion The contraceptive effects of immediate application of Jenefox intrauterine device after induced abortion and application of Jenefox intrauterine device after menstruation are similar, the successful rates of the two methods are high, the operation is simple, the adverse reactions are few, but immediate application of Jenefox intrauterine device after induced abortion can avoid the secondary pain of intrauterine operation.
