中心静脉穿刺管穿刺置管持续引流低位盆腔脓肿的疗效分析

摘　　要：目的:探讨中心静脉穿刺管穿刺置管持续引流对低位盆腔脓肿的临床治疗效果。方法:选取2016年12月—2019年12月我院妇科收治的78例低位盆腔脓肿患者,随机分为观察组39例,行中心静脉穿刺管穿刺置管持续引流术+抗生素冲洗。对照组39例,行阴道后穹隆切开引流术+抗生素冲洗。分别观察两组间手术时间、术中出血量、脓液引流量、手术疼痛评分、炎性指标恢复正常时间(白细胞、中性粒细胞百分比、C-反应蛋白、降钙素原)、堵管发生率、中转手术率、术后复发率的差异性。结果:观察组及对照组在手术时间、术中出血量、脓液引流量、手术疼痛评分、炎性指标恢复正常时间、中转手术率、术后复发率方面有统计学意义(P<0.05);而在堵管发生率间的差别无统计学意义(P>0.05)。结论:中心静脉穿刺管穿刺置管持续引流在治疗低位盆腔脓肿中创伤小,引流范围广,可多脓腔引流,降低了脓肿复发率及中转腹腔镜或开腹手术机率,值得临床推广应用。Objective:To investigate the clinical effect of central venipuncture continuous drainage in the treatment of low pelvic abscess.Methods:A total of 39 patients with low pelvic abscess admitted to our hospital from December 2016 to December 2019 who received central venipuncture continuous drainage and antibiotic were collected as the observation group,and another 39 patients who received incision and drainage of the posterior vault of the vagina during the same period were included as the control group.The operative time,intraoperative blood loss,drainage volume of absess,surgical pain score,time for the inflammatory indicators to return to normal levels(including counts of white blood cells,neutrophils,serum C-reactive protein and procalcitonin),the incidence of tube blockage,the operation rate,and the postoperative recurrence rate were observed.Results:There were statistically significant differences between the two groups in terms of operation time,intraoperative blood loss,drainage volume of absess,surgical pain score,time for inflammatory indicators to return to normal levels,operation rate,and postoperative recurrence rate(P<0.05),while there were no statistically significant difference between the incidence of tube blockage(P>0.05).Conclusion:Central venipuncture continuous drainage is a new method for the treatment of low pelvic abscess with the advantages of less trauma,wide drainage range,and multiple abscesses drainage,reduces the recurrence rate of abscess and the rate of laparoscopy or laparotomy,and is worthy of clinical application.
