张仲景脾胃思想在儿科肺系疾病诊治过程中的体现

摘　　要：结合《伤寒杂病论》中张仲景重视脾胃的思想,提出重视脾胃应贯穿于儿科肺系疾病诊治的全过程。对于肺系疾病的诊断应重视望诊和趺阳脉诊,通过明确胃气的强弱来判断正气的强弱、邪气的盛衰以及疾病的转归;辨证时更要重视脾肺的关系,确定是否伴有脾胃虚损或夹食夹积等情况;治疗上应在调和营卫的基础上兼顾调理脾胃,以达到补土生金、提高疗效、预防恶变之效;在服药时及药后调护方面同样要顾护脾胃,以防过汗而损伤正气或饮食不当内损脾胃,有碍疾病的康复;此外,通过观察患儿的饮食情况,还可以判断胃气的有无,以明确疾病的预后。Combined with ZHANG Zhongjing’s thought of paying attention to the spleen and stomach in Treatise on Cold Damage and Miscellaneous Diseases,it is proposed that paying attention to the spleen and stomach should span the whole course of diagnosis and treatment of pediatric pulmonary system diseases.For the diagnosis of pulmonary system diseases, we should attach importance to inspection and anterior tibial pulse diagnosis, and judge the strength and weakness of healthy qi, exuberance and decline of pathogenic factors and the prognosis of the disease by clarifying the strength and weakness of stomach qi.We should pay more attention to the relationship between spleen and lung in syndrome differentiation, and determine whether it is accompanied by consumption of spleen and stomach or dyspepsia and retention of food.On the basis of regulating yingqi and weiqi, we should also give consideration to regulate the spleen and stomach, so as to achieve the efficacy of invigorating earth and generating metal, improving efficacy and preventing malignant transformation.In the aspects of taking medicinal and recuperation after medication, we should protect and nurse the spleen and stomach, so as to prevent internal damage of the spleen and stomach due either to excessive sweat or to improper diet, either of which hinders the recovery of diseases.In addition, by observing the diet of children patients, we can also judge whether there is stomach qi, so as to clarify the prognosis of the disease.
