摘  要:目的通过对儿童人工耳蜗植入患者进行术后开机调试及行为测试测得阈值(T值)和最大舒适阈(C值)的测试,探讨人工耳蜗植入术后T值和C值的变化趋势及其相关因素,为人工耳蜗术后调试及开机调试提供便捷。方法收集皖南医学院附属弋矶山医院行诺尔康国产人工耳蜗植入术且单侧植入的患者76例作为研究对象,术后按时开机并调试,定期随访至1年。按术前有无佩戴助听器分为助听器组和无助听器组,按照植入人工耳蜗时的年龄将研究对象分组,≤3岁为第1组,3~7岁为第2组,>7岁为第3组,对以上各组分别在术后1个月开机、开机后3个月、6个月、1年完成开机调试,记录24个电极的T值与C值,取1、7、13、19、24电极的平均值。结果各组儿童人工耳蜗植入术后在开机不同时间点C值和T值差异均有统计学意义(P均<0.05)。随着康复时间延长,T值和C值越来越大;开机后不同时间C值组间差异有统计学意义(P均<0.05),两两比较第1组、第2组C值高于第3组。在开机后6个月与1年,组间差异有统计学意义(P均<0.05),第1组、第2组T值高于第3组。相对较早植入国产人工耳蜗,术后开机调试T值和C值较大;助听器组和无助听器组开机后3个月、6个月、1年比较,助听器组C值高于无助听器组,差异有统计学意义(P均<0.05)。开机后3个月,助听器组T值高于无助听器组,差异有统计学意义(P<0.05)。术前佩戴助听器,T值和C值相对较大。结论在随访的1年时间内,随着开机后康复时间的增加T值和C值随之增大。越早进行手术的患者,T值和C值相对较大。术前佩戴助听器有利于术后开机调试的适应。Objective To investigate the response threshold and maximum comfort of domestic cochlear implants after a postoperative start-up,behavioral response threshold(T-levels)and maximum comfort threshold(C-levels)test in children coch-lear implant patients.The trend of threshold changes and related factors provide more postoperative debugging data for domestic cochlear implants,which provides conven-ience for postoperative commissioning of adolescent cochlear implant recipients.Methods Seventy-six patients who underwent Norconk domestic cochlear implant and were unilaterally implanted at the Yijishan Hospital affiliated to Wannan Medical College were on time and commissioned for one year.All the implanted patients were grouped according to the age of cochlear implant,divided into≤3 years old group;3<age≤7 years old group;7<age≤16 years old group,grouped according to pre-operative hearing aids.Debugging were completed for each group at the time of switch-on 1 month,3 months,6 months and 1 year after switch-on.Record the T and C values of 24 electrodes,take 1,7,13,19,The average of 24 electrodes.Results There were statistically significant differences in C-levels and T-levels at different time points after cochlear implantation in each group(all P<0.05).With the prolonged recovery time,the response threshold and maximum comfort threshold were larger and larger;There were statistically significant differences in C-levels between groups at different time after startup(all P<0.05),and pairwise comparison results showed that The C-levels in group 1 and group 2 was higher than that in group 3.At 6 months and 1 year after starting up,the difference between groups was statistically significant(P<0.05),and the T-levels in groups 1 and 2 was higher than that in group 3.Relatively early implantation of domestic cochlear implants,T-levels and C-levels were large;The C-levels of the hearing aid group was higher than that of the hearing aid group at 3 months,6 months and 1 year after starting,and the difference was statisticall
