磺脲类降糖药物失效后2型糖尿病患者经瑞格列奈联合精蛋白生物合成人胰岛素注射液(预混30R)治疗的疗效评价

摘　　要：目的探究瑞格列奈联合精蛋白生物合成人胰岛素注射液(预混30R)治疗磺脲类降糖药物失效2型糖尿病(T2DM)患者的疗效。方法选取2017年12月-2020年5月偃师市中医院96例磺脲类降糖药物失效T2DM患者作为研究对象,简单随机化分两组,各48例。对照组给予预混30R治疗,观察组于对照组基础上加用瑞格列奈。对比两组疗效及治疗前后血糖指标[糖化血紅蛋白(HbA1c)、空腹血糖(PG)、餐后2h血糖(2h PC)].血糖变异指数[最大血糖波动幅度(LAGE).血糖值标准差.(SDBG)]及血清分泌型卷曲相关蛋白-5(SFRP5)、视黄醇结合蛋-4(RBP-4)水平。结果观察组总有效率95.83%高于对照纽的77.08%(P<0.05);治疗后观察组PPG、2hPG、HbA1c水平低于对照组(P<0.05);治疗后观察组LAGE、SDBG低于对照组(P<0.05);治疗后观察组血清SFRP5水平高于对照组,血清RBP-4水平低于对照组(P<0.05).结论瑞格列奈联合预混30R治疗磺脲类降糖药物失效T2DM患者,可调节血清SFRP5、RBP-4表达,提高血糖控制效果,降低血糖变异指数,效果显著。0bjective To invesigate the eficacy of repaglinide combined with primine biosynhetic human insuin iection(premixed 30R)in the treatment of type 2 diabetles mllitus(T2DM)with failure of sulfonylurea hypogyemic drugs.Methods From December 2017 to May 2020,96 T2DM patients with failure of sulfonylureas hypoglycemic drugs in our hospital were slected as the research objets.They were simply randomized into groups,48 cases in each group.The control group was given premixed 30R treatment,and the observation group was given repaglinide on the basis of the control group.Compared two groups of curative efet and treatment before and afer blood sugar index[glycated hemoglobin(HbA1,asting plasma glucose(FP),2ch posprandial blood glucose(2h PC),and glucose vaiablity index[maximum blood sugar fluctuation(LAGE),standard deviation(SDBC)]in blood sugar levels and secrein of serum type curl-PRP5),etinol binding proleien involved in egs-4(RBP)lel.Results The lotal efecive rate of obervaion group was 95.83%higher.than that of control group(7.08%,P<0.05).After treatment,he levels of FPG,2hPG and HBALC in observation group were lower than those in contol group P<05).After treatmen,LAGE and SDBG of observation group were lower than those of control group(P<0.05).After treatment,he serum SFRPS level of obsevation group was higher than that of control group,and the seum RBP 4 level of obseration group was lower than that of control group(P<05).Conc lus ion In the teatment of T2DM palients with filure of sulfonylurea hypoglycemie drug,repaginide combined with premixed 30R can regulate serum SFRP5 and RBP 4 expesion,improve the efet of blood glucose control,and reduce the glycemic variation index,with signifcant eft.
