摘　　要：目的:探讨约束决策轮对急诊重症监护室(Emergency Intensive Care Unit,EICU)患者身体约束情况的应用效果。方法:选择2017年9月—2019年2月南通大学附属医院收治的EICU患者为研究对象,按照入院时间将患者分为干预组(n=199)和对照组(n=157),干预组通过约束决策轮评估对患者实施必要性身体约束,对照组通过《约束护理单》对患者实施身体约束。结果:两组身体约束时间和身体约束率比较差异均有统计学意义(均P<0.05);干预组非计划性拔管率虽有降低,但差异无统计学意义(P>0.05);两组患者约束期均无皮肤损伤。结论:应用约束决策轮对EICU患者进行身体约束评估,可明显减少约束时间,有效降低身体约束率,保障患者的安全。Objective:To explore the application effect of constraint decision wheel on physical constraint of Emergency Intensive Care Unit(EICU)patients.Methods:EICU patients of the Affiliated Hospital of Nantong University from September 2017 to February 2019 were selected as the research objects.They were divided into intervention group(n=199)and control group(n=157)according to the admission time.The intervention group implemented necessary physical constraints on patients through constraint decision wheel evaluation,and the control group implemented physical constraints on patients through constraint nursing sheet.Results:There were significant differences in body restraint time and body restraint rate between the two groups(both P<0.05).In terms of unplanned extubation rate,although the intervention group decreased,the difference was not statistically significant(P>0.05).There was no skin injury in the two groups during the restraint period.Conclusion:The application of constraint decision wheel to evaluate physical restraint of EICU patients can significantly reduce constraint time,effectively reduce the rate of physical restraint,and ensure the safety of patients.
