摘  要:目的探讨腹腔镜经胃腔切除(laparoscopic intragastric resection,LIGR)并保留贲门的胃肠道间质瘤(gastrointestinal stromal tumor,GIST)微创化手术治疗效果。方法回顾性分析2019年2月至2020年6月广东省中医院胃肠外科行LIGR贲门周围3例GIST病人的临床特征、手术方法及预后。结果3例病人均顺利完成LIGR手术,手术时间为120~180 min,术中出血量为10~80 mL,术中、术后无并发症发生,术后随访6~22个月,病人肿瘤无复发或转移。结论LIGR治疗贲门周围的内生型GIST是一种安全、有效、合理、微创的手术方式,可以最大限度地保留正常胃组织,提高了保留贲门的可能性,减少了贲门狭窄、穿孔、出血等并发症。Objective To explore the efficacy of mini-invasive surgery of gastric cardia preservation by laparoscopic transgastric resection(LIGR).Methods Clinical features,surgical approaches and outcomes were retrospectively reviewed for 3 patients with gastrointestinal stromal tumor(GIST)around cardia undergoing LIGR.Results All 3 patients completed LIGR successfully.The operative duration was 120-180 min and the intraoperative volume of blood loss 10-80 mL.No postoperative complications,tumor recurrence or metastasis occurred during a follow-up period of 6-22 months.Conclusion As a safe,effective,reasonable and mini-invasive surgery for endogenic GIST around cardia,LIGR can maximize the retention of normal gastric tissue,improve the possibility of cardiac preservation and lower the occurrences of such complications as cardiac stenosis,perforation and hemorrhage.
