摘　　要：目的研究介入封堵治疗多发性房间隔缺损(ASD)中,三维超声心动图的应用价值。方法选取本院心脏大血管外科2017年1月至2019年6月收治的43例确诊为多发性ASD患者,43例患者均行经胸超声心动图(TTE)及经食管超声心动图(TEE)行三维成像检查,符合相关入组条件,在X线及TTE或者TEE辅助引导下行导管封堵治疗ASD,收集相关临床及随访数据。结果在43例多发性ASD中,ASD缺损形态学特点34例为双孔ASD患者,9例为筛孔型ASD。43例患者均介入导管封堵,封堵器直径14~36mm。其中双封堵器封堵9例,1例术后出现残余分流。单封堵器封堵34例,均为大孔封堵,4例术后出现残余分流。43例患者术后随访6~27个月,3例有残余分流。结论在多发ASD病例诊断、封堵器的型号选择及术后疗效评价等方面,三维超声心动图均有重要的作用。Objective:To evaluate the value of three-dimensional echocardiography in the interventional operation of multiple atrial septal defect(ASD).Methods:A total of 43 patients with ASD diagnosed in the Department of Cardiovascular Surgery of the General Hospital of Ningxia Medical University from January 2017 to June 2019 were selected to screen patients with multiple ASD using transthoracic echocardiography(TTE)and transesophageal echocardiography(TEE).Percutaneous ASD occlusion surgery was performed under X-ray and TTE or TEE observations with 3-D imaging technique,and clinical and follow-up data were collected.Results:Forty-three patients with multiple ASD,ASD defect morphological characteristics:34 cases were double-hole ASD patients,9 cases were sieve-hole ASD.The occluder was successfully occluded in all 43 patients.The diameter of the occluder was from 14mm to 36mm.Nine of them used two occluders to block two holes,and one patient had residual shunt after operation.The other 34 cases were blocked with an occluder,all of which blocked large holes,and 4 patients had residual shunt after operation.Forty-three patients were followed up for 6 to 27 months after operation.Three patients had residual shunts.Conclusion:Three-dimensional echocardiography plays an important role in the diagnosis of multiple ASD cases,the choice of occluder model and the evaluation of postoperative curative effect.
