摘 要:目的:探讨二维灰阶超声联合超微血管成像(SMI)对甲状腺影像与报告系统(Thyroid Imaging Reporting and Data System,TI-RADS)3、4级甲状腺结节良恶性的鉴别诊断价值。方法:选取2018年3月至2019年12月在本院进行二维超声和超微血管成像检查的甲状腺结节患者70例(92个结节),并对其进行中国版C-TIRADS危险分级及SMI血流灌注特征的评估,借鉴既往文献关于甲状腺良恶性病灶微血流模式的分析,以手术病理结果作为金标准,评价二维灰阶超声联合超微血管成像鉴别诊断甲状腺结节良恶性的敏感性、特异性、阳性预测值和阴性预测值。结果:与病理结果比较,二维灰阶超声联合超微血管成像评估甲状腺结节的敏感性为90.0%,特异性为92.8%,阳性预测值为93.8%,阴性预测值为88.6%。结论:二维灰阶超声联合超微血管成像对鉴别诊断TI-RADS 3、4级甲状腺结节的良恶性有一定的临床应用价值,SMI为二维超声鉴别诊断甲状腺良恶性结节提供了一种有效补充方法,值得临床推广实践。Objective:To investigate the value of two-dimensional(2D)gray-scale ultrasound combined with superb microvascular imaging(SMI)in the differential diagnosis of Thyroid Imaging Reporting and Data System(TI-RADS)3 and 4 thyroid nodules.Methods:A total of 70 patients with thyroid nodules(92 nodules)who underwent 2D ultrasound and SMI in our hospital between March 2018 and December 2019 were included,and were evaluated for risk classification by using the Chinese version of TIRADS(C-TIRADS)and patterns of blood perfusion on SMI.According to published report on patterns of microvascular blood flow in benign and malignant thyroid lesions,with surgical pathology as the gold standard,the sensitivity specificity,positive predictive value and negative predictive value of 2D gray-scale ultrasound combined with SMI in differential diagnosis of benign and malignant thyroid nodules were evaluated.Results:Compared with pathology,2D gray-scale ultrasound combined with SMI yielded a sensitivity of 90.0%,a specificity of 92.8%,a positive predictive value of 93.8%,and a negative predictive value of 88.6%,in assessment of thyroid nodules.Conclusion:2D gray-scale ultrasound combined with SMI is clinically valuable in the differential diagnosis of TI-RADS 3 and 4 thyroid nodules between benign and malignant.SMI provides a usel alternative to 2D ultrasound in distinguishing between benign and malignant thyroid nodules,and therefore warrants widespread use in clinical setting.
