摘  要:目的:研究“养元通络”针法对2型糖尿病(type 2 diabetes mellitus,T2DM)模型大鼠肝脏胰岛素抵抗相关通路指标的影响,探究其对T2DM的干预作用及可能机制。方法:将50只SD大鼠按随机数字表法分为正常组、模型组、针刺组、二甲双胍组和假针刺组5组各10只,采用链脲佐菌素腹腔注射构建2型糖尿病大鼠模型,空白组和模型组不做任何干预,针刺组给予“养元通络”针法干预,二甲双胍组给予盐酸二甲双胍灌胃,假针刺组选取非穴位区域针刺干预,4周后检测各组大鼠空腹血糖(fasting blood-glucose,FBG)和随机血糖(random blood-glucose,RBG),采用免疫印迹法检测肝组织磷脂酰肌醇-3激酶(phosphatidylinositol 3-kinase,PI3K)、蛋白激酶B(protein kinase B,AKT)的表达,采用荧光定量PCR检测肝组织胰岛素受体底物-1(insulin receptor substrate,IRS-1)、糖原合成酶激酶-3(glycogen synthase kinase 3β,GSK-3β)、腺苷酸活化蛋白激酶(AMP-activated protein kinase,AMPK)mRNA的表达。结果:与正常组比较,模型组FBG、RBG明显升高(P<0.01),肝组织PI3K、AKT蛋白表达明显降低(P<0.01),肝组织IRS-1、AMPK mRNA表达明显降低(P<0.01),GSK-3βmRNA表达明显升高(P<0.01);与模型组比较,二甲双胍组、针刺组FBG、RBG降低(P<0.05),肝组织PI3K、AKT蛋白表达升高(P<0.05),肝组织IRS-1 mRNA、AMPK mRNA表达升高(P<0.05),GSK-3βmRNA表达明显下降(P<0.05)。结论:养元通络针法能通过上调T2DM大鼠肝脏PI3K、AKT、IRS-1、AMPK表达,下调GSK-3β表达,调控肝脏胰岛素抵抗防治2型糖尿病。Objective:To research the effects of Yangyuan Tongluo acupuncture on hepatic insulin resistance related indicators in type 2 diabetes mellitus(T2DM)model rats and explore its intervention effect and possible mechanism.Methods:50 SD rats were randomly divided into 5 groups with random digital method:normal group,model group,acupuncture group,metformin group and sham acupuncture group,10 rats in each group.Type 2 diabetes rats model were established by intraperitoneal injection of streptozotocin.After 4 weeks’treatment,random blood sugar(RBG)and fasting blood sugar(FBG)were detected by blood glucose meter,PI3K、AKT were detected by Western Blot method,IRS-1、GSK-3β、AMPK mRNA were detected by Real time quantitative PCR.Results:Compared with normal group,FBG and RBG were raised obviously(P<0.01),the expression levels of hepatic PI3K,AKT were declined(P<0.01),the expression level of hepatic IRS-1、AMPK mRNA were declined(P<0.01),while GSK-3βmRNA was raised in model group(P<0.01).Compared with model group,FBG,RBG were declined(P<0.05),the expression levels of hepatic PI3K,AKT were raised(P<0.05),the expression levels of hepatic IRS-1 mRNA、AMPK mRNA were raised(P<0.05),while hepatic GSK-3βmRNA was declined in metformin and acupuncture group(P<0.05).Conclusion:Yangyuan Tongluo acupuncture could regulate hepatic insulin resistance and intervene type 2 diabetes by up-regulating the expression of PI3K、AKT、IRS-1、AMPK and down-regulating the expression of GSK-3βin T2DM rats.
