背俞指针疗法对GERD大鼠胃起搏区MLCK及P_(MLC20)表达水平的影响

摘　　要：目的:探讨背俞指针疗法对胃食管反流病(gastroesophageal reflux disease,GERD)大鼠食管黏膜及胃起搏区组织肌球蛋白轻链激酶(myosin light chain kinase,MLCK)和磷酸化肌球蛋白调节性轻链(phosphorylation 20kD Myosin light chain phosphorylation,P_(MLC20))表达的影响。方法:SD大鼠按照随机数字表法分为空白组、模型组、西药组(枸橼酸莫沙必利分散片联合兰索拉唑肠溶片)和指针组,采用HE染色观察大鼠食管中下段组织的病理变化,荧光定量PCR法检测MLCK mRNA表达,免疫组化法检测MLCK、P_(MLC20)蛋白表达。结果:与空白组比较,模型组食管鳞状上皮基底细胞中度增生,基底细胞层增厚,固有层乳头延长,固有层内炎细胞浸润,细胞间隙增宽;与模型组比较,指针组和西药组食管黏膜结构基本恢复正常。模型组MLCK mRNA、蛋白及P_(MLC20)表达均明显低于空白组(P<0.05),指针组MLCK mRNA、蛋白及P_(MLC20)表达均明显高于模型组(P<0.05或P<0.01),且指针组MLCK mRNA表达水平明显高于西药组(P<0.05),但MLCK蛋白表达水平明显低于西药组(P<0.05)。结论:GERD的发病与胃起搏区组织的MLCK mRNA、蛋白及P_(MLC20)表达降低有关,背俞指针疗法治疗GERD疗效确切,其机制可能与其上调MLCK mRNA、蛋白及P_(MLC20)表达有关。Objectives:To investigate the effect of Finger Acupuncture on Back-Shu Point’s on esophageal mucosa and expression levels of myosin light chain kinase(MLCK)and 20kD myosin light chain phosphorylation(P_(MLC20))in gastric pacing region of Gastroesophageal reflux disease(GERD)rats.Methods:According to the random number table method,SD rats were randomly divided into blank group,model group,western medicine group(mosapride citrate dispersible tablets combined with lansoprazole enteric-coated tablets)and finger acupuncture group,each group took intervention for 14 days.After the interventions,the pathological changes of the middle and lower esophagus tissues of rats were observed by light microscopy;the expression levels of MLCK mRNA in gastric pacing region of rats were detected by RT-qPCR method;the expression levels of MLCK and P MLC 20 in this region were detected by immunohistochemistry method.Results:Compared with the blank group,the model group showed moderate hyperplasia of basal cells in esophageal squamous epithelium,thickening of basal cell layer,prolongation of lamina propria,infiltration of inflammatory cells in the lamina propria,and widening of cell gap.Compared with the model group,the mucosa structure of the finger acupuncture and the western medicine group basically returned to normal.The expression levels of MLCK mRNA,MLCK and P_(MLC20) in model group were lower than those in blank group(P<0.05);the expression levels of MLCK mRNA,MLCK and P_(MLC20) in finger acupuncture group were higher than those in model group(P<0.05 or P<0.01);the expression level of MLCK mRNA in the western medicine group was lower than that in the finger acupuncture group(P<0.05),while the expression level of MLCK protein in the western medicine group was higher than that in the finger acupuncture group(P<0.05).Conclusions:The onset of GERD was related to the decreased expression levels of MLCK mRNA,MLCK and P_(MLC20) protein in the gastric pacing area.The effect of Finger Acupuncture on Back-Shu Point on GERD is definite,a
