涤痰止眩汤治疗原发性高血压(痰湿壅盛型)的临床疗效及对血脂水平的调节作用

摘　　要：目的:观察涤痰止眩汤治疗原发性高血压(痰湿壅盛型)的临床疗效及对血脂水平的调节作用。方法:以2018年3月~2020年3月来四川省八一康复中心就诊的原发性高血压患者96例为研究对象,依据随机对照法将患者分为研究组(n=48例)和对照组(n=48例)。对照组患者口服硝苯地平缓释片,每次30mg,每日1次。研究组口服硝苯地平缓释片+涤痰止眩汤治疗。两组分别治疗4周。比较研究组和对照组患者治疗前后中医症候积分变化,比较两组前后血压指标(SBP、DBP)和血脂指标(TC、TG、HDL-C);比较两组治疗前后简明健康状况量表(SF-36)评分,比较两组治疗疗效。结果:研究组治疗总有效率较对照组高(P<0.05);研究组治疗后眩晕、头痛、胸闷、呕吐痰诞和纳呆等中医症候积分低于对照组(P<0.05);研究组治疗后SBP、DBP、TC、TG水平低于对照组,HDL-C高于对照组(P<0.05);对照组治疗后睡眠状况和焦虑状况评分高于研究组(P<0.05)。结论:涤痰止眩汤结合西药常规治疗原发性高血压疗效显著,能显著降低血压和血脂水平,提高患者的生活质量,提升治疗疗效,值得临床借鉴。Objective: To study the clinical efficacy of using Ditan Zhixuan Decoction in the treatment of primary hypertension(excessive accumulation of phlegm-dampness type) and its regulation of blood lipid level. Methods: 96 patients with essential hypertension who came to Sichuan Provincial Bayi Rehabilitation Center from March 2018 to March 2020 were selected as the research objects. According to the random control method, the patients were divided into a study group(n=48) and a controlled group(n=48). Patients in the controlled group were treated with Nifedipine Sustained-release Tablets, 30 mg/time, once a day. The study group was treated with Nifedipine Sustained-release Tablets and Ditan Zhixuan Decoction. The two groups were treated for 4 weeks. The changes of TCM symptom scores, blood pressure indexes(SBP,DBP) and blood lipid indexes(TC,TG,HDL-C) before and after treatment, and the scores of SF-36 before and after treatment were compared between the two groups. Results: The curative effect of the controlled group was higher than that of the study group(P < 0.05). The scores of traditional Chinese medicine symptoms such as vertigo, headache, chest tightness, vomiting, phlegm and natant were lower in the study group than in the controlled group(P < 0.05). After treatment, the levels of SBP,DBP,TC and TG in the controlled group were higher than those in the study group, while HDL-C in the controlled group was lower than that in the study group(P < 0.05). The scores of sleep and anxiety in the controlled group were higher than those in the study group after treatment(P < 0.05). Conclusion: The combination of Ditan Zhixuan Decoction and normal western medicine has a significant effect in the treatment of essential hypertension, which can significantly improve the patients’ clinical symptoms, reduce blood pressure and blood lipid levels, improve the patients’ quality of life, improve the therapeutic effect, and is worthy of clinical reference.
