摘  要:目的:观察复合空间模式下踝背屈训练与功能性电刺激对偏瘫患者步行能力的影响。方法:对2016年7月—2018年4月在医院接受康复治疗的脑卒中偏瘫患者156例,采取随机数字法分为常规训练组(A组)、功能性电刺激组(B组)、踝背屈训练与功能性电刺激组(C组)三组;三组均给予常规康复训练,B组加予功能性电刺激训练,C组加予复合空间模式下踝背屈训练与功能性电刺激,三组疗程均为8周。采用Holden步行能力分级(FAC)、下肢简式Fugl-Meyer运动评分(FMA)、Berg平衡量表评分(BBS)和改良Barthel指数(MBI)分别评定患者的步行能力、下肢运动水平、平衡功能和生活活动水平。结果:①三组各有2例无法坚持训练,退出研究,每组有50例完成研究。A组、B组和C组的总有效率分别为80.0%、90.0%,98.0%;差异显著(P<0.05);②经治疗三组Holden步行能力分级均比之前改良(P<0.01),B组和C组改良程度优于A组(P<0.05);③治疗后3组的FMA评分、BBS评分和MBI评分有不同水平升高(P<0.01),C组升高幅度最大,B组次之。结论:采取复合空间模式下踝背屈训练与功能性电刺激能有效地改良脑卒中偏瘫患者的踝背屈功能和步行时的异常模式,有利于促使患者的步行能力、下肢运动功能、平衡功能和ADL能力的恢复。Objective:To observe the effects of ankle dorsiflexion training in compound space mode and functional electrical stimulation on walking ability of hemiplegic patients.Methods:156 patients with hemiplegia after stroke received rehabilitation treatment in hospital from July 2016 to April 2018 were randomly divided into three groups:the routine training group(group A),the functional electrical stimulation group(group B),the ankle dorsiflexion training and functional electrical stimulation group(group C).Three groups were given routine rehabilitation training,group B was added to give functional electrical stimulation training,and group C was added to give ankle dorsiflexion training in compound space mode and functional electrical stimulation,the treatment lasted for 8 weeks in three groups.Holden walking ability scale(FAC),Fugl-Meyer motor score(FMA),Berg balance scale score(BBS)and modified Barthel index(MBI)were used to evaluate the walking ability,lower limb motor function,balance function and ADL ability of patients.Results:Two patients of each group could not persist in training and quit the study,and 50 patients in each group completed the study.The total effective rates of group A,group B and group C were 80.0%,90.0%and 98.0%respectively,with significant difference(P<0.05).Holden walking ability of three groups was improved(P<0.01),and the improvement degree of group B and group C were better than that of group A(P<0.05).FMA score,BBS score and MBI score of three groups increased at different levels after treatment(P<0.01),and group C increased the most,followed by group B.Conclusion:Ankle dorsiflexion training and functional electrical stimulation can effectively improve ankle dorsiflexion function and abnormal walking pattern in stroke patients with hemiplegia,and promote the recovery of walking ability,lower limb motor function,balance function and ADL ability.
