从络脉理论探讨心绞痛的诊疗思路

摘　　要：心绞痛是由于冠状动脉供血不足或心肌耗氧量增加所致心肌暂时性缺血、缺氧,以胸部闷痛、压榨性疼痛为主要临床表现的疾病,是冠状动脉粥样硬化性心脏病的最常见表现,临床以西药及血管重建为主要治疗方法。冠状动脉粥样硬化性心脏病心绞痛属于中医的"胸痹""心痛"范畴,随着中医理论体系的发展,历史悠久的络脉理论逐渐显露其特有的优势。从络脉立论,心绞痛多以"络脉虚滞"为基本病理变化,病理性质多为本虚标实,因此临证治疗中虚者重在补益心之气血阴阳虚损;滞者重在疏通血瘀、痰浊、毒邪之阻滞,从而收获较好的临床疗效。Angina pectoris is a disease characterized by temporary ischemia and hypoxia of the myocardium caused by coronary artery blood shortage or increased myocardial oxygen consumption,the main clinical manifestations of the disease are chest pain and compression pain.It is the most common manifestation of coronary heart disease.Western medicine and vascular reconstruction are the main clinical treatment method.Coronary heart disease angina pectoris belongs to the category of "chest pain" and "heartache" of traditional Chinese medicine.With the development of theoretical system of traditional Chinese medicine,the long-established theory of collaterals gradually reveals its unique advantages.From the perspective of collaterals theory,the basic pathological change of angina pectoris is "deficiency and excess of collaterals",and the pathological properties are mostly the original deficiency and the superficial solidarity.Therefore,in the treatment of clinical syndrome,the deficiency focuses on replenishing the deficiency of Qi,Blood and Yin and Yang of the heart.The stagnation focusing on clearing blood stasis,phlegm turbidities and toxic evil obstruction,so as to obtain better clinical efficacy.
