不同严重程度阻塞性睡眠呼吸暂停低通气综合征夜间血压波动特点分析

摘　　要：目的:分析不同严重程度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)夜间血压波动特点。方法:选取100例OSAHS患者,依据血氧饱和度(SaO_(2))值与呼吸暂停低通气指数(AHI)分为轻度组(n=40)、中度组(n=36)和重度组(n=24)。比较3组睡眠参数及血压参数,分析不同严重程度OSAHS患者夜间血压波动特点。结果:经Pearson分析,OSAHS患者的AHI、ODI、MAI、TST90与△BP、血压波动频次呈正相关(r>0,P<0.05),LSpO_(2)、MSpO_(2)与△BP、血压波动频次呈负相关(r<0,P<0.05);经线性回归分析显示,中重度OSAHS与夜间血压波动有关(P<0.05)。结论:OSAHS患者夜间血压波动与病情严重程度具有密切相关。Objective:To analyze the characteristics of nocturnal blood pressure fluctuation in obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:One hundred patients with OSAHS were selected as the research subject and divided into mild(n=40),moderate(n=36) and severe(n=24) groups,based on blood oxygen saturation(SaO_(2)) and apnea hypopnea index(AHI).The sleep and blood pressure parameters were compared among the three groups and the characteristics of nocturnal blood pressure fluctuation in patients with OSAHS with different severity were analyzed.Results:Pearson analysis showed that AHI,ODI,Mai and tst90 were positively while correlated with △BP and blood pressure fluctuation frequency(r>0,P<0.05),LSaO_(2) and MSaO_(2) were negatively correlated with △BP and blood pressure fluctuation frequency(r<0,P<0.05).Linear regression analysis showed that moderate and severe OSAHS were related to nocturnal blood pressure fluctuation(P<0.05).Conclusion:The nocturnal blood pressure fluctuation in OSAHS patients is closely related to the severity of the disease.
