童趣化诱导在先心病介入术后患儿中的护理效果

摘　　要：目的探讨童趣化诱导在先心病学龄前期患儿介入封堵术后配合的护理效果,为提高先心病患儿介入术后的配合率提供方案。方法选取2017年10月—2019年3月在天津市胸科医院呼吸内科接受介入治疗的先心病患儿214例为研究对象,按照随机数字表法分为研究组和对照组各107例。对照组给予常规护理干预,研究组采用视觉、听觉、语言鼓励、奖励等童趣化诱导方式使术后患儿配合患肢制动,观察比较两组患儿在术后24 h内患肢伤口出血/皮下血肿发生率、配合制动率、镇静药使用率和患儿家属满意率。结果研究组患儿伤口出血/皮下血肿发生率为6.54%,低于对照组的23.36%;研究组患儿配合平卧位≥12 h占比为81.31%,配合沙袋压迫≥6 h占比为85.05%,均高于对照组的20.56%和31.78%;研究组患儿使用镇静药物率为15.89%,低于对照组患儿的36.45%;研究组患儿家属满意率为93.46%,高于对照组的63.55%,差异均有统计学意义(P<0.05)。结论对学龄前期患儿实施童趣化诱导干预,能缓解患儿的焦虑恐惧情绪及生理应激反应,有效提高先心病介入术后患儿的配合率,从而降低介入术后穿刺部位出血/皮下血肿的发生率,提高患儿家属满意率,值得临床推广应用。Objective To explore the effect of childlike interest induction on the cooperation of preschool children with congenital heart disease after interventional closure,to provide a scheme for improving the cooperation rate of children with congenital heart disease after interventional surgery.Methods From October 2017 to March 2019,214 children with congenital heart disease who received interventional treatment in the Respiratory Department of Tianjin Chest Hospital were selected as the research objects and divided into study group and control group,with 107 case in each group according to random number table method.The control group was given routine nursing intervention,and the study group used childlike interest induction methods such as vision,hearing,language encouragement and reward to make the children cooperate with the postoperative limb braking.The incidence of wound bleeding/subcutaneous hematoma of the affected limb wound within 24 hours after operation,the cooperation braking rate,the use rate of sedatives and the satisfaction rate of the children’s family members were observed and compared between the two groups.Results The incidence of wound bleeding/subcutaneous hematoma in the study group was6.54%,which was lower than 23.36%in the control group;the proportions of the children in the study group cooperated with the supine position≥12 hours and the sandbag compression≥6 hours were 81.31%and85.05%,which were higher than 20.56%and 31.78%in the control group;the use rate of sedative drugs in the study group was 15.89%,which was lower than 36.45%in the con trol group;the satisfaction rate of children’s family members in the study group was 93.46%,which was higher than 63.55%in the control group,and the differences were statistically significant(P<0.05).Conclusion The implementation of childlike interest induction intervention for preschool children could alleviate the anxiety and fear emotion and physiological stress response of children,effectively improve the cooperation rate of children with con
