益气养阴解毒通络法治疗急性期糖尿病性脑梗死临床观察

摘　　要：目的观察益气养阴解毒通络法治疗糖尿病性脑梗死急性期患者的临床疗效,并探讨其对患者血清炎性因子水平(TNF-α、hs-CRP)的影响。方法临床收集120例糖尿病性脑梗死急性期患者,随机分为益气养阴解毒通络法(即自拟解毒降糖复瘫汤)治疗组和补阳还五汤对照组,每组60例。2组在基础治疗上,治疗组予以解毒降糖复瘫汤治疗,对照组予以传统补阳还五汤治疗,评估2组治疗前后脑卒中残损评定法评分(SIAS)、Barthel指数评分(BI)、简明精神状态检查表评分(MMSE)、炎性因子水平(TNF-α、hs-CRP)以及胰岛素敏感指数,并比较疗效差异。结果治疗1个疗程后,2组糖尿病性脑梗死急性期患者的SIAS、BI、MMSE、炎性因子水平(TNF-α、hs-CRP)和总体疗效均有改善,其中治疗后总体疗效、功能评分、TNF-α和hs-CRP方面2组间比较差异具有统计学意义(P<0.05)。结论解毒降糖复瘫汤治疗糖尿病性脑梗死急性期患者疗效明显,能提高患者治疗总有效率,改善患者神经功能和日常生活能力,调节患者血清炎性因子水平,值得临床推广应用。Objective To evaluate the clinical effect of supplementing qi for nourishing yin and removing toxic substance for dredging collaterals therapy in the treatment of diabetic cerebral infarction in acute period,and to explore the influence on the patients with serum inflammatory factors level(TNF-α,hs-CRP).Methods 120 patients with diabetic cerebral infarction in acute stage were randomly divided into a treatment group and a control group,with 60 cases in each group.On the foundation treatment in both groups,the patients in the treatment group were given supplementing qi for nourishing yin and removing toxic substance for dredging collaterals therapy(Jiedu Jiangtang Futan decoction)and the patients in the control group were given Buyang Huanwu decoction.The two groups were evaluated by stroke impairment assessment set(SIAS),Barthl index(BI),min-mental state examination(MMSE),inflammatory factor levels(TNF-αand hsCRP),Insulin sensitivity index(ISI),and the difference of curative effect was compared.Results After one course of treatment,the SIAS,BI,MMSE scores,inflammatory factor levels(TNF-α,hs-CRP)and overall efficacy of patients in the acute phase of diabetic cerebral infarction in both groups were improved,and the efficacy,functional scores,TNF-α,hs-CRP level after treatment of patients in the treatment group were significantly better than those in the control group(P<0.05).Conclusion Jiedu Jiangtang Futan decoction in the treatment of diabetic cerebral infarction in acute stage can improve the clinically curative effect,improve nerve function defect and disability degree,adjust the level of serum inflammatory factors,and is worthy of clinical promotion.
