中医情志干预对晚期食管癌疼痛患者生存质量的影响

摘　　要：目的研究中医情志护理对晚期食管癌疼痛患者生存质量的影响。方法选取2018年4月—2020年12月山西省肿瘤医院宁养院收治的晚期食管癌疼痛患者100例。按照数字随机表法分为2组,其中对照组50例采用常规护理,观察组50例患者在对照组基础上实施中医情志护理,观察并比较2组护理前后疼痛评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、睡眠质量评分,并从角色功能、情绪功能、认知功能、社会功能评分4个方面对患者的生存质量评分进行比较。结果护理前,2组患者疼痛、SAS、SDS及睡眠质量评分比较差异无统计学意义(P>0.05);护理后,观察组疼痛、SAS、SDS及睡眠质量评分明显低于对照组,差异有统计学意义(P<0.05);经过护理干预后,观察组的角色功能、情绪功能、认知功能、社会功能评分均明显高于对照组,差异有统计学意义(P<0.05)。结论采用中医情志护理干预可改善食管癌疼痛患者的负性情绪,减轻患者疼痛程度,提高生存质量,值得临床推广应用。Objective To study the effect of traditional Chinese medicine emotional nursing on the quality of life of patients with advanced esophageal cancer pain.Methods 100 cases patients with advanced esophageal cancer pain in Shanxi Provincial Cancer Hospital,in accordance with the table numbers,were randomly divided into two groups.50 cases were treated by routine nursing care as control group,and the other 50 cases were given traditional Chinese medicine emotional nursing on the basis of the control group.Patients’pain,sleep quality,the quality of survival,and bad state of mind was compared between the two groups.Results There was no difference in pain scores between the two groups at admission(P>0.05).After nursing intervention,the pain score of the control group was significantly higher than that of the observation group(P<0.05).After nursing,SAS score,SDS score and sleep quality score in observation group were lower than those in control group,and the difference was significant(P<0.05).After different nursing interventions,the scores of cognitive function,emotional function,social function and role function in the observation group were significantly higher than those in the control group,and the difference was significant(P<0.05).Conclusion The traditional Chinese medicine emotional nursing can improve the negative emotions of patients with esophageal cancer pain,reduce the pain status of patients,improve the quality of life,and is worthy of clinical application.
