心宝丸联合氯沙坦钾片治疗微血管心绞痛的疗效及对冠状动脉血流储备功能、Angptl2、Angptl4水平影响

摘　　要：目的探讨心宝丸联合氯沙坦钾片在微血管心绞痛(MVA)患者中的应用效果。方法选取2020年1月—2021年2月河北工程大学附属医院心内一科诊治MVA患者122例,根据Doll’s临床病例随机表法分为对照组和研究组,各61例。2组均给予基础治疗,对照组另予氯沙坦钾片治疗,研究组另予心宝丸+氯沙坦钾片治疗。比较2组疗效、不良反应发生率,治疗前后心绞痛症状、冠状动脉血流储备功能(CFR)、血脂、血清血管生成素样蛋白2(Angptl2)、Angptl4水平。结果研究组治疗总有效率高于对照组(91.80%vs.78.69%,χ^(2)/P=4.171/0.041);2组患者治疗后6周、12周心绞痛疼痛程度、发作次数、持续时间降低(P<0.01),且研究组优于对照组(治疗后6周:t/P=6.276/0.000、6.922/0.000、3.512/0.001;治疗后12周:t/P=6.270/0.000、5.539/0.000、10.164/0.000)。2组患者治疗后6周、12周HDL-C、CFR、Angptl4升高(P<0.01),LDL-C、TG、TC、Angptl2降低(P<0.01),且研究组改善优于对照组(治疗后6周:t/P=5.235/0.000、6.084/0.000、4.379/0.000、4.651/0.000、4.354/0.000、3.801/0.000、4.405/0.000;治疗后12周:t/P=5.513/0.000、5.883/0.000、4.833/0.000、5.833/0.000、5.019/0.000、3.631/0.000、4.674/0.000);2组治疗过程中均未出现血尿常规、凝血功能及肝肾功能异常,研究组头痛、头晕、恶心、腹泻、疲劳发生率与对照组比较,差异无统计学意义(P>0.05)。结论心宝丸联合氯沙坦钾片应用于MVA患者,可有效减轻心绞痛症状,增强CFR,调节血脂及血清Angptl2、Angptl4水平,且安全性高。Objective To investigate the effect of Xinbao pill combined with losartan potassium tablets in patients with microvascular angina pectoris(MVA).Methods One hundred and twenty two patients with MVA were selected from the First Department of Cardiology,Affiliated Hospital of Hebei University of Engineering from January 2020 to February 2021.According to doll's clinical case random table,they were divided into control group and study group,with 61 cases in each group.Both groups were given basic treatment,the control group was treated with losartan potassium tablets,and the study group was treated with Xinbao pill+losartan potassium tablets.The curative effect,incidence of adverse reactions,symptoms of angina pectoris,coronary flow reserve function(CFR),blood lipid,serum angiopoietin like protein 2(Angptl2)and ANGPTL4 levels were compared between the two groups before and after treatment.Results The total effective rate of the study group was higher than that of the control group(91.80%vs.78.69%,χ^(2)/P=4.171/0.041)。The pain degree,attack frequency and duration of angina pectoris in the two groups decreased 6 and 12 weeks after treatment(P<0.01),and the study group was better than the control group(6 weeks after treatment:t/P=6.276/0.000,6.922/0.000,3.512/0.001;12 weeks after treatment:t/P=6.270/0.000,5.539/0.000,10.164/0.000).At 6 and 12 weeks after treatment,HDL-C,CFR and Angptl4 increased(P<0.01),LDL-C,TG,TC and Angptl2 decreased(P<0.01),and the improvement of the study group was better than that of the control group(6 weeks after treatment:t/P=5.235/0.000,6.084/0.000,4.379/0.000,4.651/0.000,4.354/0.000,3.801/0.000,4.405/0.000;12 weeks after treatment:t/P=5.513/0.000,5.883/0.000,4.833/0.000,5.833/0.000,5.019/0.000,3.631/0.000,4.674/0.000)There was no abnormal blood and urine routine,coagulation function,liver and kidney function in the two groups during treatment.There was no significant difference in the incidence of headache,dizziness,nausea,diarrhea and fatigue between the study group and the control group(
