摘 要:目的:探究动力型支具训练在手屈肌腱断裂修复术后患者早期康复中的应用效果及对手功能恢复的影响。方法:选取2017年3月—2019年12月于天津医院行手屈肌腱断裂修复术的60例患者作为观察对象,根据随机数字表法分为对照组和观察组各30例。对照组行常规干预,观察组在对照组基础上行动力型支具训练。比较两组患者干预12个月后的肌腱疗效、手握力和捏力、不良事件及生活质量情况;结果:观察组总主动活动度(TAM)评分显著高于对照组,对照组临床总有效率为63.33%,低于观察组(86.67%),差异有统计学意义(P<0.05);对照组不良事件发生率为53.33%,高于观察组(23.33%),差异有统计学意义(P<0.05);与干预前比较,干预后两组患者握力、三点捏力、侧捏力均显著升高,且观察组升高幅度显著大于对照组,差异有统计学意义(P<0.05);干预12个月后,观察组简明健康状况量表(SF-36)评分显著高于对照组,差异有统计学意义(P<0.05)。结论:动力型支具训练在手屈肌腱断裂修复术后患者早期康复中取得了显著的应用效果,对功能恢复起到了促进作用,且不良事件少,值得临床推广应用。Objective To explore the application effect of dynamic brace training in early rehabilitation of patients with hand flexor tendon rupture and its influence on functional recovery.Methods 60 patients with hand flexor tendon rupture in our hospital from March 2017 to December 2019 were selected and randomly divided into control group(n=30)and observation group(n=30).The control group received routine intervention while the observation group received movement support training on the basis of routine intervention.The total active movement(TAM)score,clinical efficacy,hand grip and pinch strength before and after intervention,the incidence of adverse events and the life quality were compared between the two groups after 12 months of intervention.Results The TAM score of the observation group was significantly higher than that of the control group.The clinical total effective rate of the control group was 63.33%and the observation group was 86.67%,which the difference was statistically significant(P<0.05).The adverse events rate of the control group was 53.33%,which was higher than 23.33%of the observation group(P<0.05).Compared with those before intervention,the grip strength,three-point pinch strength and side pinch strength of the two groups were significantly increased after intervention,and the increase range of the observation group was significantly higher than that of the control group(P<0.05).After 12 months of intervention,the SF-36 score of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The dynamic brace training in the early rehabilitation of patients with hand flexor tendon rupture has achieved a more significant application effect,which plays a role in promoting the functional recovery,with fewer adverse events and has a high clinical reference value.
