清热化痰汤对高血压左心室肥厚所致的室性期前收缩患者的临床观察

摘　　要：目的探讨清热化痰汤对高血压左心室肥厚所致的室性期前收缩患者的临床观察。方法选取86例高血压左心室肥厚所致的室性期前收缩患者,按随机数字表法分为观察组和对照组各43例。对照组采用常规西药治疗,观察组在对照组基础上采用清热化痰汤治疗。观察比较2组临床疗效,治疗前后24 h室性期前收缩的总次数、平均心率、心率震荡(HRT)指标及左室质量指数(LVMI)的变化情况,并评价2组患者用药安全性。结果观察组治疗后总有效率为90.7%,显著高于对照组的72.1%(P<0.05);2组治疗后24 h室性期前收缩的总次数、24 h平均心率、男性和女性LVMI、震荡初始(TO)指标均明显降低(P<0.05),且观察组降低程度更为明显(P<0.05);2组治疗后振荡斜率(TS)、震荡斜率起始时间(TT)均明显升高(P<0.05),且观察组升高程度更为明显(P<0.05);2组患者治疗前后基本生命体征指标、血常规、尿常规、大便常规、肝功能、肾功能、电解质均无发生明显异常,治疗期间也未出现明显不良反应。结论清热化痰汤对高血压左心室肥厚所致的室性期前收缩治疗效果良好,能够调节心率震荡指标,减少室性期前收缩次数,改善左室肥厚,值得临床深入研究。Objective To elucidate the clinical efficacy of Qingre Huatan Decoction on patients with premature ventricular contraction caused by hypertensive left ventricular hypertrophy.Methods A total of 86 patients with ventricular extrasystole caused by hypertensive left ventricular hypertrophy were selected and randomly divided them into observation group and control group with 43 cases in each group.The peitents in control group were treated with conventional western medicine,while the ones in observation group were treated with Qingre Huatan Decoction on the basis of the control group.The clinical efficacy,the total number of 24-hour ventricular extrasystoles,the average heart rate,the index of heart rate turbulence(HRT)and the change of left ventricular mass index(LVMI)before and after treatment were compared between the two groups,and the medication safety of the two groups was evaluated.Results After treatment,the total effective rate in observation group was 90.7%,which was significantly higher than that in control group(72.1%,P<0.05).The total number of ventricular extrasystoles in 24 hours,the average heart rate in 24 hours,LVMI and turbulence oneset(TO)indexes of men and women in both groups decreased significantly(P<0.05),especially in the observation group(P<0.05).The index of turbulence slope(TS)and turbulence timing(TT)in both groups increased significantly(P<0.05),especially in the observation group(P<0.05).There were no obvious abnormalities in basic vital signs,blood routine,urine routine,stool routine,liver function,kidney function and electrolytes before and after treatment,and no apparent adverse reactions occurred during treatment.Conclusion Qingre Huatan decoction has a good therapeutic effect on ventricular extrasystole caused by hypertensive left ventricular hypertrophy,which can regulate the index of heart rate turbulence,reduce the number of ventricular premature contractions and improve left ventricular hypertrophy,and is worthy of further clinical research.
