摘　　要：目的探讨通过微创生物束带阴道缩窄结合开放式会阴体成型的复合式阴道紧缩术。方法从作者手术项目组2019年1月至2020年12月期间实施的复合式阴道紧缩术案例中,随机选择30例作为观察对象。首先在导引弯钩辅助下,按“阶梯倒U形”手术设计在阴道中、下段侧后壁置入、收紧生物束带以缩窄阴道腔,然后行会阴体成型术以收紧阴道口和加固会阴体。统计平均手术时间,观察阴道腔缩窄和阴道口闭拢、会阴体饱满和高度变化,以及出血、直肠损伤、伤口愈合不良等并发症;术后3月,随访求美者对外阴形状及性生活改善的满意度。结果本组30例复合式阴道紧缩术手术时间平均为2.0h;术后即刻阴道腔中、下段均明显缩窄,阴道周径由术前3横指以上缩窄至2横指以下;小阴唇由术前自然张开变为术后自然闭合;会阴体由塌陷变得饱满,会阴高度平均抬高1.0cm;本组未出现明显并发症。术后3月随访,求美者对外阴形状及性生活满意度平均93%。结论复合式阴道紧缩术采用微创结合开放式手术模式,通过生物束带缩窄阴道腔和会阴成型收紧阴道口及加强会阴体,提高了阴道紧缩的整体效果,降低了手术操作难度和手术风险,有利于巩固收紧效果和延长维持时间,值得临床推广应用。Objective To explore the combined vaginal tightening by minimally invasive narrowing vaginal with biological band and perineal molding surgery.Methods 30 cases of combined vaginal tightening operated by this surgical team from January 2019 to December 2020 were randomly selected as study subjects.First,with the assistance of the guide hook,the biological band was placed into the rear wall of the middle and lower wall of vagina according to the"ladder inverted u-shaped"surgical design in order to narrow the vaginal cavity,and then perineal molding was performed to tighten the vaginal mouth and strengthen the perineum.The mean surgical time was counted,observing complications such as vaginal cavity narrowing and vaginal mouth closing,perineal fullness and height change,as well as bleeding,rectal injury,and poor wound healing.Follow-up patient satisfaction with vulva shape and improved sexual life.Results The average surgical time of 30 combined vaginal tightening cases in this group was 2.0h.Immediately after surgery,the middle and lower vaginal segments were significantly narrowed,and the pervagaginal diameter was narrowed from above 3 transverse fingers to below 2 transverse fingers.The small labia was changed from natural preoperative opening to natural postoperative closure.The perineal body becomes full from collapse,with its height raised by an average of 1.0cm.No significant complications were observed in this group.At 3 months of postoperative follow-up,the average satisfaction with vulva shape and sexual life was 93%.Conclusion Combined vaginal tightening through minimally invasive and open surgery,biological band was used to narrow vaginal cavity and perineum forming to tighten vaginal mouth and strengthen the perineum.This kind of suegery could improve the overall effect of vaginal tightening,reduce the difficulty of surgical operation and surgical risk,consolidate the tightening effect and extend maintenance time.It suggested that this suegery is worthy of clinical promotion and application.
