摘 要:目的:观察与分析下肢手术切口感染患者足甲襞微循环及血流变状态的变化情况。方法:选取2018年1月-2020年9月的50例下肢手术切口感染患者为观察组,同时期未发生切口感染的50例患者为对照组。检测与比较两组足甲襞微循环指标(加权积分、输入及输出枝长度、白微栓率及管襻畸形率)及血流变指标(各类血液黏度及红细胞指数),同时比较观察组不同感染情况患者的检测指标,并采用Pearson相关性分析足甲襞微循环及血流变指标与下肢手术切口感染的关系。结果:观察组输入及输出枝长度均显著短于对照组,其他足甲襞微循环指标及血流变指标均显著高于对照组(P<0.05),且观察组不同感染情况患者检测指标比较,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,输入及输出枝长度与下肢手术切口感染均呈负相关(P<0.05),其他足甲襞微循环指标及血流变指标与下肢手术切口感染均呈正相关(P<0.05)。结论:下肢手术切口感染患者足甲襞微循环及血流变状态呈现显著异常,与切口感染有密切的关系,因此在本类患者中的检测价值较高。Objective:To observe and analyze the change situation of foot nailfold microcirculation and hemorheology state of patients with incision infection of lower extremity surgery.Method:From January 2018 to September 2020,50 patients with incision infection of lower extremity surgery were chosen as the observation group,50 patients without incision infection at the same time were chosen as the control group.Then the foot nailfold microcirculation indexes(weighted integral,length of input and output branches,white microthrombus rate and loop deformity rate)and hemorheology indexes(various blood viscosity and erythrocyte indexes)of two groups were compared,and the detection indexes of observation group with different infection situation were compared,then the correlation between the foot nailfold microcirculation indexes,hemorheology indexes and incision infection of lower extremity surgery were analyzed by Pearson correlation analysis.Result:The length of input and output branches of the observation group were significantly shorter than those of the control group,other foot nailfold microcirculation indexes and hemorheology indexes were significantly higher than those of the control group(P<0.05),and the detection indexes of the observation group with different infection situation were compared,there were statistically significant differences(P<0.05).Pearson correlation analysis showed the length of input and output branches were negatively correlated with incision infection of lower extremity surgery(P<0.05),other foot nailfold microcirculation indexes and hemorheology indexes were positively correlated with incision infection of lower extremity surgery(P<0.05).Conclusion:The foot nailfold microcirculation and hemorheology state of patients with incision infection of lower extremity surgery show significantly abnormal state,and they have close relationship to the incision infection,so the detection value in these patients are higher.
