摘　　要：目的:研究自体富血小板血浆治疗糖尿病患者难治性创面的效果。方法:选取2019年9月-2020年12月本院收治的糖尿病难治性创面患者180例为研究对象,根据随机数字表法将其分为对照组和观察组,每组90例。对照组应用常规疗法进行创面处理,观察组在此基础上应用自体富血小板血浆治疗。比较两组创面愈合时间、治疗效果和生长因子水平。结果:观察组治疗总有效率为95.56%,高于对照组的85.56%,差异有统计学意义(P<0.05)。观察组创面愈合时间短于对照组,差异有统计学意义(P<0.05)。治疗2个周期后,观察组VEGF、TGF-β1、bFGF水平均高于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:自体富血小板血浆在糖尿病患者的难治性创面治疗中应用效果较好,有助于促进创面愈合,可能通过提高生长因子水平等机制来促进创面愈合,且安全性较高。Objective:To study the effect of autologous platelet-rich plasma in the treatment of refractory wounds in diabetic patients.Method:A total of 180 patients with diabetic refractory wounds admitted to our hospital from September 2019 to December 2020 were selected as the research objects,according to random number table method,they were divided into control group and observation group,90 cases in each group.The control group was treated with conventional therapies for wound treatment,and the observation group was treated with autologous platelet-rich plasma on this basis.Wound healing time,therapeutic effect and growth factor levels were compared between the two groups.Result:The total effective rate of observation group was 95.56%,higher than 85.56%of control group,the difference was statistically significant(P<0.05).The wound healing time of the observation group was shorter than that of the control group,the difference was statistically significant(P<0.05).After 2 cycles of treatment,the levels of VEGF,TGF-β1 and bFGF in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Autologous platelet-rich plasma has a good application effect in the treatment of refractory wounds in diabetic patients,which is helpful to promote wound healing and may promote wound healing by increasing the level of growth factors and other mechanisms,with high safety.
