摘  要:目的:分析血脂水平与早产儿生长发育指标的关系及对新生儿呼吸窘迫综合征(RDS)的预测价值。方法:选取2019年1月-2020年12月于本院分娩的158例早产儿为观察组,另选同期的158例健康足月儿为对照组。在出生后12 h内检测总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平。比较两组生长发育指标(胎龄、体重、身长、胸围、腹围、头围、上臂周径、肩宽)。使用Pearson相关性分析血脂水平与早产儿生长发育指标的关系。观察RDS发生情况,通过受试者工作特征曲线(ROC)下面积(AUC)评价血脂对RDS的预测效能。结果:两组出生身长、胸围、腹围、头围、上臂周径、肩宽比较,差异均无统计学意义(P>0.05);观察组胎龄、出生体重均低于对照组,差异均有统计学意义(P<0.05)。两组TC、LDL-C和HDL-C水平比较,差异均无统计学意义(P>0.05)。观察组TG低于对照组,差异有统计学意义(P<0.05)。早产儿TG水平与胎龄、出生体重均呈正相关(P<0.05)。RDS发生率为60.76%(96/158)。RDS组TG、胎龄、出生体重均低于非RDS组,差异均有统计学意义(P<0.05)。TG预测早产儿发生RDS的AUC为0.912,最佳截断值为0.27 mmol/L,敏感度为91.63%,特异度为68.35%。胎龄的AUC为0.665,最佳截断值为31.25周,敏感度为71.23%,特异度为53.42%。出生体重的AUC为0.580,最佳截断值为1.53 kg,敏感度为63.45%,特异度为28.63%。TG检测早产儿发生RDS的AUC均高于胎龄和体重(P<0.05)。结论:TG水平与早产儿胎龄及出生体重密切相关,其预测RDS发生的效能较好,值得临床予以重视。Objective:To analyze the relationship between blood lipid levels and growth and development indexes of premature infants and the predictive value of neonatal respiratory distress syndrome(RDS).Method:A total of 158 premature infants delivered in our hospital from January 2019 to December 2020 were selected as the observation group,and 158 healthy full-term infants in the same period were selected as the control group.Total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)and high density lipoprotein cholesterol(HDL-C)levels were detected within 12 h after birth.Growth and development indexes(gestational age,body weight,body length,chest circumference,abdominal circumference,head circumference,upper arm circumference and shoulder width)were compared between two groups.Pearson correlation was used to analyze the relationship between blood lipid levels and growth and development indexes of premature infants.The incidence of RDS was observed,and the area under receiver operating characteristic curve(AUC)was used to evaluate the predictive efficacy of blood lipid on RDS.Result:There were no significant differences in birth length,chest circumference,abdominal circumference,head circumference,upper arm circumference and shoulder width between two groups(P>0.05).The gestational age and birth weight of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).There were no significant differences in TC,LDL-C and HDL-C levels between two groups(P>0.05).TG in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).TG level was positively correlated with gestational age and birth weight(P<0.05).RDS incidence was 60.76%(96/158).TG,gestational age and birth weight in the RDS group were lower than those in the non-RDS group,the differences were statistically significant(P<0.05).The AUC of TG prediction of RDS in premature infants was 0.912,and the optimal cut-off value was 0.27 mmo
