摘　　要：目的:探究心肌梗死患者miR-23a-3p的表达情况及其临床意义。方法:选取2019年1月-2020年1月60例ST段抬高型心肌梗死(STEMI)患者为观察组,选取同期的50例非STEMI胸痛患者为对照组,对其进行回顾性分析。检测与比较两组的血清miR-23a-3p、B型脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)及C反应蛋白(CRP)水平,比较观察组中不同全球急性冠状动脉事件注册评分(GRACE评分)及预后情况(存活及死亡)患者的检测结果,采用Pearson相关性分析miR-23a-3p与BNP、cTnI及CRP的关系。结果:观察组血清miR-23a-3p水平显著低于对照组,血清BNP、cTnI及CRP水平均显著高于对照组,且观察组中不同GRACE评分及预后患者的血清miR-23a-3p、BNP、cTnI及CRP水平比较,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,血清miR-23a-3p与血清BNP、cTnI及CRP均呈负相关(P<0.05)。结论:心肌梗死患者血清miR-23a-3p的表达水平显著降低,且不同危险程度及预后患者的差异显著,并与疾病相关指标有密切的关系,因此在心肌梗死患者中的检测价值相对较高。Objective:To investigate the expression situation of miR-23a-3p of patients with myocardial infarction and its clinical significance.Method:A total of 60 patients with ST-segment elevation myocardial infarction(STEMI)from January 2019 to January 2020 were chosen as the observation group,50 patients with chest pain and without STEMI at the same time were chosen as the control group,and the data were analyzed retrospectively.Then the serum miR-23a-3p,B-type natriuretic peptide(BNP),cardiac troponin I(cTnI)and C-reactive protein(CRP)levels of two groups were detected and compared,then the detection results of observation group with different global registry of acute coronary events(GRACE)score and prognosis situation(survival and death)were compared,then the relationship between serum miR-23a-3p and serum BNP,cTnI,CRP levels were analyzed with Pearson correlation analysis.Result:The serum miR-23a-3p level of the observation group was significantly lower than that of the control group,the serum BNP,cTnI and CRP levels were all significantly higher than those of the control group,the serum miR-23a-3p,BNP,cTnI and CRP levels of the observation group with different GRACE score and prognosis situation were compared,there were statistically significant differences(P<0.05).The Pearson correlation analysis showed that serum miR-23a-3p were negatively correlated with serum BNP,cTnI and CRP(P<0.05).Conclusion:The expression levels of serum miR-23a-3p of patients with myocardial infarction is significantly decreased,and the differences of patients with different risk degree are significant,and it has close relationship to the disease related indexes,so the detection value in the patients with myocardial infarction is relatively higher.
