T细胞酶联免疫斑点法对艾滋病合并结核病患者的诊断价值

摘　　要：目的针对T细胞酶联免疫斑点法在艾滋病合并结核患者诊断中的应用价值进行分析。方法选取2019年1月至2020年9月阳江市公共卫生医院收治的160例结核病患者作为研究对象,根据临床症状、实验室及影像学检查、病理学结果等进行分组。艾滋病合并结核病患者为研究组,共96例;结核感染患者为对照组,共64例。所有数据均应用SPSS 20.0统计学软件开展分析工作。对两组患者进行T细胞酶联免疫斑点法(T-SPOT.TB)检测,结核菌素试验(PPD试验)以及抗HIV、CD4+T淋巴细胞检查,比较T-SPOT.TB检测、PPD试验的诊断敏感性和特异性。结果经检测诊断后,研究组的T-SPOT.TB阳性率显著高于对照组(P<0.05)。比较T-SPOT.TB与PPD试验的诊断价值,T-SPOT.TB的敏感性、特异性、阳性预测值均高于PPD试验,阴性预测值项目评价低于PPD试验(P<0.05)。不同CD4+T淋巴细胞计数患者的T-SPOT.TB、PPD试验阳性率比较,差异无统计学意义(P>0.05)。结论T细胞酶联免疫斑点法对艾滋病合并结核病患者具有较高的诊断效能,有利于诊断和鉴别该疾病。Objective To analyze the application value of T cell enzyme-linked immunospot assay in the diagnosis of patients with acquired immune deficiency syndrome(AIDS)combined with tuberculosis.Methods A total of 160 patients with tuberculosis admitted to Yangjiang Public Health Hospital from January 2019 to September 2020 were selected as study subjects and grouped according to clinical symptoms,laboratory and imaging examinations,pathological findings,etc.96 patients with AIDS combined with tuberculosis were selected as the study group,and 64 patients with tuberculosis infection were selected as the control group.All data were analyzed statistically by SPSS 20.0 statistical software.The T cell enzyme-linked immunospot assay(T-SPOT.TB)test,tuberculin test(PPD test)and anti-HIV,CD4+T lymphocyte examination were performed in both groups,and the diagnostic sensitivity and specificity of T-SPOT.TB test and PPD test were compared.Results According to the diagnosis by the tests,the positive rate of T-SPOT.TB in the study group was significantly higher than that in the control group(P<0.05).In terms of the diagnostic value of T-SPOT.TB and PPD test,the evaluation on sensitivity,specificity and positive predictive value of T-SPOT.TB were higher than those of PPD test,while the item evaluation on negative predictive value of T-SPOT.TB was lower than that of PPD test(P<0.05).Besides,there were no statistically significant differences between T-SPOT.TB and PPD test in the positive rate in patients with different CD4+T lymphocyte counts(P>0.05).Conclusion The T cell enzyme-linked immunospot assay has a high diagnostic efficacy in patients with AIDS combined with tuberculosis and is beneficial for diagnosis and identification of the disease.
