摘 要:目的探究重症肺炎患儿血清及诱导痰液细胞因子动态变化与预后之间相关性。方法选取2019年1月至2020年6月通辽市医院儿科收治的120例重症肺炎患儿的一般资料进行回顾性研究,并按照患儿病程第7天转归情况及疗效判断标准将其划分两组,第一组为好转组(n=60),第二组为恶化组(n=60),两组患儿均采用ELISA法以及诱导痰液技术对患儿血清和痰液标本中的白介素6(IL-6)、白介素10(IL-10)以及肿瘤坏死因子(TNF-α)进行测定,并通过重症肺炎预后PORT评分系统(PSI)对两组患者的预后情况进行评价,以上指标均于患儿病程第1、3、7天进行动态监测,并对各病程时段检测所得的炎症细胞因子水平与PSI评分之间的相关性进行分析和研究。结果两组病程第1天的血清和痰液标本中的IL-6、IL-10、TNF-α以及PSI评分比较,差异无统计学意义(P>0.05);好转组病程第3天和第7天的血清和痰液标本中的IL-6、IL-10、TNF-α以及PSI评分均低于恶化组,差异有统计学意义(P<0.05),重症肺炎患儿血清及诱导痰液细胞因子水平与PSI评分呈现明显正相关。结论对重症肺炎患儿血清及诱导痰液细胞因子进行动态变化监测,能够有效判断病情进展,并为患者治疗及预后提供有效依据,应予以推广。Objective To investigate the correlation between dynamic changes of serum and induced sputum cytokines and prognosis in children with severe pneumonia.Methods The general data of 120 children with severe pneumonia admitted to the Department of Pediatrics of Tongliao Hospital from January 2019 to June 2020 were collected for a retrospective study.These children were divided into the improving group(n=60)and the deteriorating group(n=60)according to their recoverycondition on day 7 of the disease course and the judgment criteria of curative effect.The interleukin-6(IL-6),interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α)in serum and sputum specimens of both groupswere measured by the enzyme linked immunosorbent assay(ELISA)and the induced sputum technique.The prognosis of the two groups was evaluated by the severe pneumonia patient outcomes research team(PORT)scoring system(pneumonia severity index(PSI)).The above indicators were monitored dynamically on day 1,3 and 7 of the disease course,and the correlation between the levels of inflammatory cytokines and the PSI scores was analyzed and studied.Results There were no significant differences between the two groups in the IL-6,IL-10 and TNF-αlevels in serum and sputum specimens,as well as PSI scores on day 1 of the disease course(P>0.05).The IL-6,IL-10 and TNF-αlevels in serum and sputum specimens,as well as PSI scores on days 3 and 7 of the disease course in the improving group were lower than those in the deteriorating group,with statistically significant differences(P<0.05),serum and induced sputum cytokine levels were significantly positively correlated with PSI scores in children with severe pneumonia.Conclusion The monitoring on the dynamic changes of serum and induced sputum cytokines in children with severe pneumonia can effectively determine the progress of the disease and provide an effective basis for treatment and prognosis of patients,which should be promoted.
