摘　　要：目的探讨分析拘禁球囊技术(JBT)在老年冠状动脉分叉病变经皮冠状动脉介入治疗(PCI)血管保护中的作用。方法选取2019年1月至2020年5月于广东省罗定市人民医院就诊的60例冠状动脉分叉病变行PCI治疗患者,采用简单随机化分组法将其分为两组,观察组(n=30)接受JBT技术保护血管、对照组(n=30)接受传统血管保护方法,比较两组患者术中一般情况、术中及术后并发症发生情况,并对主要不良心血管事件进行随访1年,比较两种技术的临床疗效。结果两组术中一般情况比较,观察组手术时间、对比剂剂量、支架植入数量及放射线用量均低于对照组,手术成功率高于对照组(P<0.05);两组术中并发症发生情况比较,观察组并发症总发生率低于对照组(P<0.05);两组1年内不良心血管事件发生率比较,观察组明显低于对照组(P<0.05)。结论冠状动脉分叉病变患者行PCI术中应用JBT技术,可缩短手术时间,提高手术成功率,减少X线使用,减少术中并发症的发生及术后不良心血管事件的发生,减轻PCI术对血管的损伤。Objective To investigate and analyze the role of jailed balloon technique(JBT)in vascular protection of percutaneous coronary intervention(PCI)for coronary bifurcation lesions in the elderly.Methods A total of 60 patients with coronary bifurcation lesions attending Luoding People's Hospital of Guangdong Province for receiving PCI treatment from January 2019 to May 2020 were selected as the study subjects.They were divided into two groups using the simple randomization method,the observation group(n=30)received JBT for vascular protection and the control group(n=30)received traditional vascular protection methods.The general conditions during the operation,the occurrence of complications during and after the operation were compared between the two groups,and the major adverse cardiovascular events were followed up for one year.The clinical efficacies of the two techniques were also compared.Results In terms of the general conditions during the operation,the operating duration,contrast dose,number of stents implanted and amount of radioactive rays used in the observation group were shorter/lower than those in the control group,while the success rate of operation in the observation group was higher than that in the control group(P<0.05).Comparing the incidence of intraoperative complications between the two groups,the total incidence of complications in the observation group was lower than that in the control group(P<0.05).In addition,the incidence of adverse cardiovascular events in the observation group was significantly lower than that in the control group during 1 year of follow-up(P<0.05).Conclusion The application of JBT during PCI in patients with coronary bifurcation lesions can shorten the operating duration,improve the success rate of operation,reduce the use of X-ray and the occurrence of intraoperative complications,as well as reduce the occurrence of adverse cardiovascular events after operation and the damage to blood v essels by PCI.
