骶骨倾斜角和髂嵴连线对下腰椎间盘突出症发病的影响

摘　　要：目的探讨骶骨倾斜角和髂嵴高度对下腰椎间盘突出症发病的影响。方法对本院骨科2016年1月~2019年12月收治的100例单纯L_(4-5)或L_(5)-S_(1)腰椎间盘突出症患者进行回顾性分析。分别测量并比较两组患者的骶骨倾斜角和髂嵴连线高度。骶骨倾斜角在腰椎CT正中矢状位上测量,两侧髂嵴连线高度于腰椎正位X线片上测量,将L_(4-5)椎间隙平均分为3等份,L_(4)和L_(5)椎体各自平均分为4等份,L_(4-5)椎间隙中1/3赋值0,上方为正值1~5,下方为负值-1~-5。结果两组骶骨倾斜角和髂嵴高度比较有显著差异,L_(4-5)组骶骨倾斜角和髂嵴高度显著高于L_(5)-S_(1)组(P<0.05)。结论高髂嵴、骶骨倾斜角度大的人群易发生L_(4-5)椎间盘突出;反之,易发生L_(5)-S_(1)椎间盘突出。Objective To investigate the influence of sacral slope(SS)and the height of intercrestal line(ICL)on the incidence of lower lumbar disc herniation.Methods From January 2016 to December 2019,100 cases with single level disc herniation involving L_(4-5)or L_(5)-S_(1)level treated in the department of orthopedics in our hospital were retrospectively analyzed.The angles of SS and the heights of ICL were measured and compared between the two groups.The SS was measured on the median sagittal CT images of the lumbar spine.The heights of ICL were measured on the anteroposterior X-ray plates of lumbar spine.The L_(4-5)space was divided into 3 equal parts and the L_(4)and L_(5)vertebrae were divided into four equal parts.The middle 1/3 of L_(4-5)space was assigned to 0,the positive value waS_(1)~5 above,and the negative value-1~-5 below.Results There were significant differences in SS and ICL between the two groups.The SS angle and sacral height were higher in L_(4-5)group,and lower in L_(5)-S_(1)group(P<0.05).Conclusion People with high iliac crest and high sacral inclination are prone to have L_(4-5)disc herniation,and conversely are prone to have L_(5)-S_(1)disc herniation.
