不同频率电针治疗带状疱疹后遗神经痛的临床疗效及对血清白细胞介素10、CXC趋化因子配体10的影响

摘　　要：目的观察不同频率电针治疗带状疱疹后遗神经痛(PHN)的临床疗效及对血清白细胞介素10(IL-10)、CXC趋化因子配体10(CXCL10)水平的影响。方法将60例PHN患者按照随机数字表法分为2组。2组均给予西医常规治疗,低频组30例予低频(2 Hz)电针治疗,高频组30例予高频(100 Hz)电针治疗。2组均治疗2周。比较2组治疗前后疼痛视觉模拟评分(VAS)、匹兹堡睡眠质量指数(PSQI)评分、疼痛阈值(PPT)及血清IL-10、CXCL10水平,并统计2组疗效。结果低频组总有效率96.67%(29/30),高频组总有效率80.00%(24/30),低频组疗效优于高频组(P<0.05)。治疗后2组疼痛VAS、PSQI评分均较本组治疗前降低(P<0.05),PPT均升高(P<0.05),且治疗后低频组疼痛VAS、PSQI评分均低于高频组(P<0.05),PPT高于高频组(P<0.05)。治疗后2组血清IL-10、CXCL10水平均较本组治疗前降低(P<0.05),且治疗后低频组低于高频组(P<0.05)。结论电针治疗PHN具有良好的辅助治疗效果,且低频电针较高频电针临床疗效更优,能改善睡眠质量,提升PPT,降低血清IL-10、CXCL10水平,免疫调节能力更佳。Objective To explore the effect of different frequency electroacupuncture on postherpetic neuralgia(PHN)and its effect on influence on Serum Interleukin-10(IL-10)and chemokine IP-10(CXCL10)levels.Methods A total 60 PHN patients were randomly divided into two equal groups.In addition of conventional Western medicine,low-frequency group was given low-frequency electroacupuncture(2 Hz),and high-frequency group given high-frequency electroacupuncture(100 Hz).Both groups were treated for 2 weeks in order to compare visual analog scale(VAS),Pittsburgh Sleep Quality Index(PSQI),pressure pain threshold(PPT),serum levels of IL-10 and CXCL10,as well as efficacy between groups before and after treatment.Results After treatment,total response in low-frequency group was better than in high-frequency group[96.67%(29/30)vs 80.00%(24/30),(P<0.05),respectively].VAS score and PSQI score in groups were decreased,and PPT was increased,VAS score and PSQI score were decreased in low-frequency group than in high-frequency group,and the PPT was higher than that in high-frequency group(P<0.05).Serum levels of IL-10 and CXCL10 were decreased in groups,which was decreased in low-frequency group than in high-frequency group(P<0.05).Conclusion Electroacupuncture therapy has a good adjuvant treatment effect for PHN,low-frequency electroacupuncture have better clinical effects versus higher-frequency.Electroacupuncture therapy can improve sleep quality,increase PPT,reduce serum IL-10,CXCL10,with better immune regulation.
