摘  要:目的·分析特发性中枢性性早熟(idiopathic central precocious puberty,ICPP)女童不饱和游离脂肪酸谱的变化及其与体质量指数(body mass index,BMI)、性激素水平的相关性。方法·选取2020年8月—2021年1月至上海交通大学附属儿童医院住院的56例ICPP女童及46例同龄正常未发育女童为研究对象,分别纳入ICPP组和对照组。收集2组女童的身高、体质量,计算BMI及体质量指数标准差评分(body mass index standard deviation score,BMI SDS);检测所有女童的血清不饱和游离脂肪酸谱的浓度,检测ICPP组女童的血清黄体生成素(luteinizing hormone,LH)、促卵泡生成激素(follicle stimulating hormone,FSH)的基础值和峰值,以及雌二醇(estradiol,E2)、性激素结合球蛋白(sex hormone-binding globulin,SHBG)、胰岛素样生长因子-1 (insulin-like growth factor-1,IGF-1)水平。对接受促性腺激素释放激素类似物(gonadotropin releasing-hormone analogues,GnRHa)治疗的女童随访3个月,比较完成随访的48例ICPP女童治疗前后血清不饱和游离脂肪酸谱的浓度。采用多元逐步回归分析血清不饱和游离脂肪酸谱与各项指标的相关性。结果·(1)ICPP组女童的BMI、BMI SDS高于对照组(P<0.05),但各种不饱和游离脂肪酸与BMI均无相关性(均P>0.05)。(2)与对照组相比,ICPP组的花生四烯酸(arachidonic acid,AA;P=0.001)、n-6二十二碳五烯酸(n-6 docosapentenoic acid,n-6DPA;P=0.000)、n-3二十二碳五烯酸(n-3 docosapentenoic acid,n-3DPA;P=0.005)、二十碳五烯酸(eicosapentaenoic acid,EPA;P=0.010)、二十二碳六烯酸(docosahexaenoic acid,DHA;P=0.003)、总n-6多不饱和游离脂肪酸(n-6 polyunsaturated fatty acids,n-6PUFA;P=0.049)水平明显降低,差异具有统计学意义。(3)油酸(oleic acid,C18:1n-9;P=0.036)、巨头鲸鱼酸(gondoic acid,C20:1n-9;P=0.030)、亚油酸(linoleic acid,LA;P=0.020)、γ-亚麻酸(γ-linolenic acid,GLA;P=0.010)、α-亚麻酸(α-linolenic acid,ALA;P=0.009)及总n-6PUFA (P=0.041)水平,�Objective·To analyze the changes of unsaturated free fatty acid profiles and their correlations with body mass index(BMI) and sex hormones in idiopathic central precocious puberty(ICPP) girls. Methods·Fifty-six ICPP girls who were hospitalized in Children’s Hospital of Shanghai Jiao Tong University from August 2020 to January 2021 and 46 normal undeveloped girls at the same age were selected, respectively included in the ICPP group and the control group. The height and weight of the two groups were collected, and the BMI and body mass index standard deviation score(BMI SDS)were calculated. The concentration of serum unsaturated free fatty acid profiles of all the girls as well as the basic and peak values of serum luteinizing hormone(LH) and follicle stimulating hormone(FSH), estradiol(E2), sex hormone-binding globulin(SHBG), and insulin-like growth factor-1(IGF-1) in ICPP girls were detected. The girls who received gonadotropin releasing-hormone analogues(GnRHa) treatment were followed up for 3 months. Finally,48 ICPP girls were enrolled in the follow-up study. The concentrations of serum unsaturated free fatty acid profiles before and after treatment in 48 ICPP girls who completed the follow-up were compared. Multivariate stepwise regression was used to analyze the correlation between serum unsaturated free fatty acid profiles and various indicators. Results·(1) BMI and BMI SDS of the ICPP group were higher than those of the control group(P<0.05), but there was no correlation between the levels of various unsaturated free fatty acids and BMI(P>0.05).(2) Compared with the control group, the levels of arachidonic acid(AA, P=0.001), n-6 docosapentenoic acid(n-6 DPA, P=0.000), n-3 docosapentaenoic acid(n-3 DPA, P=0.005), eicosapentaenoic acid(EPA, P=0.010), docosapentaenoic acid(DHA, P=0.003) and total n-6 polyunsaturated fatty acids(n-6 PUFA, P=0.049) were significantly reduced.(3)The levels of oleic acid(C18:1 n-9, P=0.036), gondoic acid(C20:1 n-9, P=0.030), linoleic acid(LA, P=0.020), γ-Linolenic acid(GLA,
