CEUS与FNA联合诊断甲状腺良恶性结节的价值分析

摘　　要：目的探讨超声造影(CEUS)与细针穿刺细胞学检查(FNA)联合诊断甲状腺良恶性结节的价值。方法选取本院2018年1月至2019年12月收治的甲状腺结节患者124例作为研究对象,均接受CEUS和FNA检查,以病理学检查为金标准,比较CEUS、FNA单独和两者联合诊断甲状腺良恶性结节的价值。结果以病理学检查结果为金标准,124例患者中共检测出136个甲状腺结节,其中甲状腺良性结节共28个,占20.59%,甲状腺恶性结节共108个,占79.41%,FNA联合CEUS诊断甲状腺良恶性结节的敏感度、特异度、阳性预测值及阴性预测值均高于单独FNA、CEUS检测(P<0.05),且与金标准诊断结果一致性好(Kappa值=0.932,P<0.05)。结论与CEUS、FNA单独检查相比,CEUS与FNA联合诊断甲状腺良恶性结节可提高诊断价值,值得临床推广应用。Objective To explore the value of contrast-enhanced ultrasound(CEUS)combined fine-needle aspiration cytology(FNA)in the diagnosis of benign and malignant thyroid nodules.Methods 124 patients with thyroid nodules admitted to our hospital from January 2018 to December 2019 were selected as the research subjects.All of them underwent CEUS and FNA examinations.Using pathological examination as the gold standard,compare the value of CEUS and FNA alone and the combination of the two in diagnosing benign and malignant thyroid nodules.Results Taking pathological examination results were as the gold standard,136 thyroid nodules were detected in 124 patients,of which 28 were benign thyroid nodules,accounting for 20.59%,and there were 108 malignant thyroid nodules,accounting for 79.41%,the sensitivity,specificity,positive predictive value and negative predictive value of CEUS combined with FNA in diagnosing benign and malignant thyroid nodules were all higher than those of FNA and CEUS alone(P<0.05),and it was consistent with the gold standard diagnosis results(Kappa value=0.932,P<0.05).Conclusion Compared with CEUS and FNA alone,the combined diagnosis of benign and malignant thyroid nodules by CEUS and FNA can improve the diagnostic value and it is worthy of clinical application.
