基于黄元御“一气周流”理论探析消渴目病

摘　　要：消渴病是糖尿病的中医病名,消渴病日久会对目窍产生影响而发为消渴目病。本文基于黄元御的“一气周流”理论,认为消渴目病的病因病机可概括为木气郁滞、土虚湿困、坎阳亏虚,致使一气运行不畅,清阳不升,日久痰积血瘀阻于目络。清阳上升于目,离不开脾土左旋、肝木升发顺畅和肾阳的充足。在治疗上,从疏肝、健脾、温肾三方面入手,中焦为一气周流的枢纽,故首治湿气困脾,健脾祛湿以助中焦升降有序,多用党参、黄芪、茯苓等药;疏肝解郁以调畅全身气机,可用柴胡、白芍等药;温补肾水以化痰祛瘀,可用附子等。本文详细阐述了一气周流的生理、病理模式,根据消渴目病的临床特征、病程进展,从一气周流理论探析其病因病机以及辨证施治,为治疗消渴目病开拓思路。Diabetes is the name of diabetes mellitus in Traditional Chinese Medicine.Diabetes mellitus will affect eyes for a long time and become diabetic eye disease.Based on HUANG Yuanyu’s theory of"one-qi circumfluence",this paper holds that the etiology and pathogenesis of diabetic eye disease can be summarized as stagnation of liver qi,insufficiency of the spleen with overabundance of dampness,deficiency of kidney-yang,which leads to unsmooth operation of one qi,lucid yang failling to rise,and stagnation of phlegm and blood stasis in eye collaterals.Lucid yang rises to the eyes,which can not be separated from the rising of spleen qi from the left,the smoothness of liver qi,and the sufficiency of kidney yang.In terms of treatment,it starts from three aspects:soothing the liver,strengthening the spleen and warming the kidney.The middle jiao is the hub of the one-qi circumfluence.Therefore,the spleen dampness should be treated first,and invigorating spleen to eliminate dampness can help the middle jiao rise and fall orderly.Salvia miltiorrhiza,Astragalus membranaceus,and Poria cocos are often used.Dispersing stagnated liver qi to relieve depression can regulate the whole body’s qi activity,and Radix bupleuri,Radix paeoniae alba can be used.Warming kidney-yang to eliminate phlegm and blood stasis can be used with aconite.In this paper,the physiological and pathological patterns of one-qi circumfluence are elaborated in detail.According to the clinical characteristics and course progress of diabetic eye disease,the etiology and pathogenesis of diabetic eye disease and syndrome differentiation and treatment are analyzed from the theory of one-qi circumfluence,so as to open up new ideas for treating diabetic eye disease.
