化痰祛瘀方联合硝苯地平对高血压中医证候、血液流变学及血栓前状态的影响

摘　　要：目的探讨化痰祛瘀方联合硝苯地平对高血压病患者中医证候、血液流变学及血栓前状态的影响。方法选取诊治的高血压病患者120例,随机分为对照组和观察组,每组60例,两组患者均给予硝苯地平片口服,观察组在以上用药基础上联合化痰祛瘀方治疗,均连续治疗4周,观察患者治疗前后中医证候积分变化,血液流变学指标血浆黏度(MP)、高切全血黏度(MH)、低切全血黏度(ML)、红细胞压积(HCT)、红细胞聚集指数(AI)变化,血栓前状态相关指标血小板a颗粒膜蛋白(GMP)、血管性血友病因子(vWF)、D二聚体(D-D)、凝血酶原激活物抑制剂-1(PAI-1)变化,血压变化。结果两组治疗前中医证候积分、血液流变学、血栓前状态指标及血压水平比较差异均无统计学意义(P>0.05);两组治疗后中医证候积分低于治疗前,且观察组低于对照组,比较差异有统计学意义(P<0.05);两组治疗后MP、MH、ML、HCT、AI水平低于治疗前,且观察组低于对照组,比较差异有统计学意义(P<0.05);两组治疗后GMP、vWF、D-D、PAI-1水平低于治疗前,且观察组低于对照组,比较差异有统计学意义(P<0.05);两组治疗后收缩压及舒张压水平低于治疗前,且观察组低于对照组,比较差异有统计学意义(P<0.05)。结论化痰祛瘀方联合硝苯地平治疗高血压能有助于症状的改善及促进血压降低,改善患者血液流变学指标及血栓前状态,疗效明显。Objective To discuss the effect of Huatan Quyu Formula(化痰祛瘀方)combined with nifedipine on traditional Chinese medicine symptoms(TCM),hemorheology and pre-thrombotic state of hypertension patients.Methods We collected 120 hypertension patients and randomly divided them into control group and observation group,60 cases in each group.The both groups were given nifedipine tablets orally.The observation group was treated with Huatan Quyu Formula for 4 weeks.We observed the scores of TCM syndromes before and after treatment,hemorheological indicators[plasma viscosity(MP),high cut whole blood viscosity(MH),low cut whole blood viscosity(ML),hematocrit(HCT),erythrocyte aggregation index(AI)],pre-thrombotic status-related indicators[platelet A particle membrane protein(GMP),von Willebrand factor(vWF),D-dimer(D-D),prothrombin activator inhibitor-1(PAI-1)]and blood pressure.Results There were no statistically significant differences in TCM symptom scores,hemorheological blood,pre-thrombotic status indexes or blood pressure between the two groups before treatment(P>0.05).The score of TCM syndromes in the two groups was after treatment lower than that before treatment,and that in the observation group was lower than that in the control group.The difference was statistically significant(P<0.05).The levels of MP,MH,ML,HCT and AI in the two groups were after treatment lower than those before treatment,and those in the observation group were lower than those in the control group.The difference was statistically significant(P<0.05).The levels of GMP,vWF,D-D and PAI-1 in the two groups were lower than after treatment those before treatment,and those in the observation group were lower than those in the control group.The difference was statistically significant(P<0.05).Conclusion Huatan Quyu Formula combined with nifedipine in the treatment of hypertension can help to improve symptoms and blood pressure,the hemorheology index and pre-thrombotic state of the patients,and the curative effect was obvious.
