苦碟子注射液配合氟伐他汀对冠心病心力衰竭超声参数的影响及安全性分析

摘　　要：目的探讨苦碟子注射液配合氟伐他汀对冠心病心力衰竭患者超声参数的影响及安全性。方法选取2019年1月—2020年3月医院冠心病心力衰竭患者106例,依据随机数字表法分为研究组与对照组,各53例。常规干预基础上对照组采取氟伐他汀,研究组在对照组基础上采取苦碟子注射液。统计两组临床疗效、治疗前后超声参数[心脏指数(CI)、心输出量(CO)、左室射血分数(LVEF)、左心室舒张末期内径(LVESD)、左心室收缩末期内径(LVEDD)]、生活质量(MLHFQ)及6 min步行距离(6MWT)、血清炎性因子、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、生长分化因子-15(GDF-15)水平、不良反应发生率。结果研究组总有效率(94.34%,50/53)高于对照组(81.13%,43/53)(P<0.05);治疗后研究组LVEF、CO、CI水平高于对照组,LVESD、LVEDD水平低于对照组(P<0.05);治疗后研究组6MWT大于对照组,MLHFQ分值低于对照组(P<0.05);治疗后研究组血清IL-18、CRP、TNF-α、NGAL、GDF-15水平低于对照组(P<0.05);研究组不良反应发生率(11.32%,6/53)与对照组(7.55%,4/53)间差异无统计学意义(P>0.05)。结论氟伐他汀及苦碟子注射液联合治疗冠心病心力衰竭效果显著,可有效改善患者心功能,提升运动能力及生活质量,可能与其能有效调节血清炎性反应状态、NGAL、GDF-15水平有关。Objective To investigate the effect and safety of Kudiezi Injection(苦碟子注射液)combined with fluvastatin on ultrasound parameters in patients with coronary heart disease and heart failure.Methods A total of 106 patients with coronary heart disease and heart failure in our hospital from January 2019 to March 2020 were selected and divided into study group and control group,with 53 cases in each group,according to the random number table method.On the basis of routine intervention,the control group took fluvastatin,and the study group took Kudiezi Injection on the basis of the control group.The clinical efficacy,ultrasound parameters[cardiac index(CI),cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),left ventricular end systolic diameter(LVEDD)],quality of life(MLHFQ)and 6-min walking distance(6MWT),serum inflammatory factors,neutrophil gelatinase-associated lipocalin(NGAL)and growth differentiation factor-15(GDF-15)before and after treatment,and the incidence of adverse reactions were calculated in the two groups.Results The total effective rate of the study group(94.34%,50/53)was higher than that of the control group(81.13%,43/53)(P<0.05).After treatment,the levels of LVEF,CO and CI in the study group were higher than those in the control group,and the levels of LVESD and LVEDD were lower than those in the control group(P<0.05).After treatment,the 6MWT of the study group was greater than that of the control group,and the MLHFQ score was lower than that of the control group(P<0.05).After treatment,the serum levels of IL-18,CRP,TNF-α,NGAL and GDF-15 in the study group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group(11.32%,6/53)and the control group(7.55%,4/53)(P>0.05).Conclusion The combination of fluvastatin and Kudiezi Injection has a significant effect in the treatment of coronary heart disease and heart failure,which can effectively improve the he
