森田疗法联合百合知母汤对首发抑郁症血清BDNF、DOPAC水平、生活质量、日常生活能力及疗效的影响

摘　　要：目的探讨森田疗法联合百合知母汤对首发抑郁症患者血清BDNF、DOPAC水平、生活质量、日常生活能力及疗效的影响。方法选取诊治的首发抑郁症患者80例,随机分为对照组和观察组,每组40例,采用阿戈美拉汀片及森田疗法进行治疗,观察组同时联合百合知母汤口服,治疗8周,观察患者治疗前后中医证候评分、血清脑源性神经营养因子(BDNF)及3,4一二羟基苯乙酸(DOPAC)水平变化,采用生活质量问卷(QLQL-74)和改良Barthel指数对患者生活质量和日常生活能力进行评价,采用汉密尔顿抑郁量表(HAMD)对治疗前后抑郁状况进行评价。结果两组治疗前中医证候评分比较差异无统计学意义(P>0.05),胸闷心烦、精神抑郁、不思饮食、失眠多梦评分治疗后较治疗前下降(P<0.05),且观察组低于对照组(P<0.05);两组治疗前血清BDNF、DOPAC水平比较差异无统计学意义(P>0.05),治疗后较治疗前升高(P<0.05),且观察组高于对照组(P<0.05);两组治疗前QLQL-74、Barthel指数评分比较差异无统计学意义(P>0.05),治疗后较治疗前升高(P<0.05),且观察组高于对照组(P<0.05);两组治疗前HAMD评分比较差异无统计学意义(P>0.05),治疗后较治疗前降低(P<0.05),且观察组低于对照组(P<0.05);观察组治疗总有效率为85.00%(34/40),高于对照组(65.00%,26/40)(P<0.05)。结论首发抑郁症患者采用森田疗法联合百合知母汤进行治疗可促进症状改善,提高生活质量及日常生活能力,疗效更佳,其作用可能与降低血清BDNF和促进DOPAC水平升高有关。Objective To discuss the effects of Morita therapy combined with Baihe Zhimu Decoction(百合知母汤)on serum brain-derived neurotrophic factor(BDNF)and 3,4-dihydroxyphenylacetic acid(DOPAC),quality of life,daily life ability and efficacy in patients with first-episode depression.Methods We collected 80 patients with first-episode depression.They were randomly divided into control group and observation group with 40 cases in each group.The patients were treated with Agomelatine Tablets and Morita therapy.The observation group at the same time combined with Baihe Zhimu Decoction orally.The treatment lasted 8 weeks.We observed the changes of TCM symptom scores,serum levels of BDNF and DOPAC before and after treatment.The quality of life questionnaire(QLQL-74)and the improved Barthel Index were used to evaluate the patients'quality of life and daily living ability.The Hamilton Depression Scale(HAMD)was used to evaluate the depression before and after treatment.Results There was no significant difference in scores of TCM syndromes between the two groups before treatment(P>0.05).The scores of chest distress,mental depression,lack of food and drink,insomnia and dreaminess after treatment were lower than those before treatment(P<0.05).The observation group's scores were lower than those of the control group(P<0.05).There was no significant difference in serum levels of BDNF or DOPAC between two groups before treatment(P>0.05).Those were higher after treatment than those before treatment(P<0.05),and the observation group's levels were higher than those of the control group(P<0.05).There was no significant difference in serum HAMD levels between the two groups before treatment(P>0.05)and they were lower after treatment than those before treatment(P<0.05),and the observation group’s levels were lower than those of the control group(P<0.05).The total effective rate in the observation group was 85.00%(34/40)and that of the control group was 65.00%(26/40).The difference was statistically significant(P<0.05).Conclusion The
