补肾调经汤治疗对多囊卵巢综合征患者卵巢超声影像学及预后的影响

摘　　要：目的探讨补肾调经汤治疗对多囊卵巢综合征(polycystic ovarian syndrome,PCOS)患者卵巢超声影像学及预后的影响。方法选取诊治的PCOS患者100例,随机分为对照组及观察组,每组50例,两组均给予相同的西药调节月经及促排卵治疗,观察组同时给予补肾调经汤口服,连续治疗3个月,观察治疗前后卵巢体积、直径2~8 mm的卵泡数量、卵巢间质动脉血流搏动指数(PI)、阻力指数(RI)、舒张末期血流速度(EVD)、收缩期峰值血流速度(PSV)变化,随访1年,观察患者治疗后的排卵及妊娠情况。结果两组治疗前卵巢体积、直径2~8 mm的卵泡数量、卵巢间质动脉血流参数比较差异无统计学意义(P>0.05),治疗后卵巢体积、直径2~8 mm的卵泡数量较治疗前明显降低(P<0.05),且观察组降低幅度大于对照组(P<0.05),治疗后PI、RI较治疗前明显升高(P<0.05),且观察组升高幅度大于对照组(P<0.05),治疗后EVD、PSV较治疗前下降(P<0.05),且观察组下降幅度大于对照组(P<0.05);观察组排卵率及妊娠率分别为82.00%(42/50)、52.00%(26/50),高于对照组64.00%(32/50)、28.00%(14/50),比较差异有统计学意义(P<0.05);观察组治疗总有效率为90.00%(45/50),高于对照组74.00%(37/50),比较差异有统计学意义(P<0.05)。结论补肾调经汤治疗后PCOS患者超声影像学上可出现明显的卵巢体积缩小及卵泡数量的减少,卵巢血流动力学改善明显,同时患者排卵及妊娠率较高,预后更佳。Objective To investigate the effect of Bushen Tiaojing Decoction(补肾调经汤)on ovarian ultrasound imaging and prognosis in patients with polycystic ovary syndrome(PCOS).Methods We collected 100 cases of patients with PCOS.They were randomly divided into control group and observation group,50 cases in each group.The two groups were treated with the same western medicine for regulating menstruation and ovulation induction.At the same time,the observation group was given Bushen Tiaojing Decoction orally for 3 months.We observed the changes of ovarian volume,number of follicles with diameter of 2-8mm,blood pulse index(PI),resistance index(RI),end-diastolic blood velocity(EVD)and systolic peak blood velocity(PSV)before and after treatment.The patients were followed up for 1 year to observe ovulation and pregnancy after treatment.Results There was no statistically significant difference in ovarian volume,number of follicles or blood flow parameters of interstitial artery between the two groups before treatment(P>0.05).The ovarian volume and number of follicles were lower than those before treatment(P<0.05)and the observation group’s ovarian volume and number of follicles was lower than those of the control group(P<0.05).PI and RI were higher than those before treatment(P<0.05)and the observation group’s PI and RI were higher than those of the control group(P<0.05).EVD and PSV were lower than those before treatment(P<0.05)and the observation group’s EVD and PSV were lower than those of the control group(P<0.05).The ovulation rate and pregnancy rate in the observation group were 82.00%(42/50)and 52.00%(26/50),which were higher than 64.00%(32/50)and 28.00%(14/50)in the control group(P<0.05).The total effective rate of the observation group was 90.00%(45/50),higher than that(74.00%,37/50)of the control group(P<0.05).Conclusion Bushen Tiaojing Decoction can improve ovarian hemodynamics,ovulation,pregnancy rate and prognosis in the treatment of PCOS.
