补胞汤结合针刺治疗卵巢储备功能下降临床观察

摘　　要：目的探讨补胞汤结合针刺法治疗卵巢储备功能下降的临床疗效以及其对相关内分泌激素、窦卵泡数、卵巢基质血流等的影响。方法选取82例2017年5月—2019年5月温州市中医院门诊就诊的卵巢储备功能下降患者,采用随机数字表法将其分成治疗组和对照组,每组41例。治疗组:补胞汤结合针刺疗法治疗,对照组:戊酸雌二醇片和地屈孕酮片周期序贯治疗。比较治疗后两组临床证候积分、卵泡刺激素(FSH)、促黄体生成素(LH)、FSH/LH,雌二醇(E2)、抗苗勒管激素(AMH)、窦卵泡数、卵巢体积、卵巢基质血流包括收缩期峰值血流速度(PSV)、搏动指数(PI)、阻力指数(RI)的差异。结果治疗3个月后,两组中医证候积分、卵泡刺激素(FSH)、促黄体生成素(LH)、FSH/LH明显下降,E_(2)、AMH明显升高,窦卵泡数增加,卵巢血供(PSV、RI、PI)有所改善,卵巢体积变化不明显;两组对比,除了FSH/LH比值差异不明显(P>0.05),余项指标改善程度差异明显(P<0.05);治疗组和对照组的总有效率分别为90.24%(37/41)和78.05%(32/41),具有统计学意义(P<0.05)。两组患者在治疗过程中出现不良反应的发生率差异无统计学意义(P>0.05)。结论运用补胞汤联合针刺能够有效的调节卵巢储备功能下降患者的内分泌激素水平,改善卵巢血供,促进卵巢储备功能恢复,安全性高,值得临床推广。Objective To explore the clinical effect of traditional Chinese medicine decoction(Bubao Decoction)(补胞汤)combined with acupuncture in patients with diminished ovarian reserve(DOR)and its influence on related female hormones,antral follicles count and ovarian stromal flow.Methods From May 2017 to May 2019,82 patients with DOR were randomly divided into treatment group and control group(41 cases in each group).The patients of the treatment group were treated with Bubao Decoction combined with acupuncture.Those of the control group were treated with sequential cycle of Estradiol Valerate Tablets and Dilprogesterone Tablets.After treatment,the symptom scores,FSH,LH,FSH/LH,estradiol(E2),anti mullerian hormone(AMH),antral follicles count(AFC),ovarian volume,and ovarian stromal flow including PSV,PI and RI were comparatively analyzed.Results After treatment of three months,the symptom scores,follicle stimulating hormone(FSH),luteinizing hormone(LH)and FSH/LH were significantly decreased in both groups.Levels of E2,AMH and AFC were significantly increased in both groups,but ovarian volume had no obvious difference.In addition to FSH/LH,the improvement of the treatment group was more obvious than that of the control group in other indicators,and the difference was statistically significant(P<0.05).The total effective rate of the treatment group was 90.24%(37/41),significantly higher than that of the control group(78.05%,32/41),and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions during treatment between the control group and the treatment group(P>0.05).Conclusion Bubao Decoction combined with acupuncture can effectively regulate the endocrine hormone levels,improve ovarian blood supply and ovarian reserve function in patients with DOR,with less adverse reactions,which is worthy of popularizing.
