摘 要:目的探讨孕期营养指导干预对妊娠期糖尿病(GDM)孕妇妊娠结局的影响。方法在广东省佛山市妇幼保健院2019年2月至2021年2月收治的GDM患者中选取80例,年龄24~35岁,孕周25~35周,体质量指数(BMI)22~28 kg/m2。采用抽签法将其分为研究组和对照组,各40例。研究组采用孕期营养干预,对照组采用常规干预。对比两组患者血糖水平、营养摄入水平、妊娠结局。结果两组干预前空腹血糖(FPG)、餐后2 h血糖(2hPBG)水平对比,差异均无统计学意义(均P>0.05);干预后,两组FPG、2hPBG均下降,研究组FPG、2hPBG分别为(8.12±1.06)ng/ml、(11.26±1.25)ng/ml,低于对照组的(9.53±1.75)ng/ml、(13.23±1.58)ng/ml(均P<0.05)。研究组维生素A、维生素E、铁、钙、锌、蛋白质摄入量分别为(953.51±100.21)μg、(182.23±19.01)μg、(36.21±3.02)mg、(753.21±58.12)mg、(19.54±3.45)mg、(98.54±12.05)g,均高于对照组的(812.12±102.05)μg、(140.20±15.24)μg、(28.10±3.24)mg、(623.10±50.30)mg、(15.02±3.02)mg、(80.15±10.12)g(均P<0.05);研究组脂肪摄入量(80.21±8.12)g,低于对照组的(92.15±6.25)g(P<0.05)。研究组不良妊娠结局发生率12.50%(5/40),低于对照组的37.50%(15/40)(P<0.05)。结论采用孕期营养指导干预在GDM孕妇中有良好效果,可有效控制血糖,有利于营养补充,改善妊娠结局。Objective To investigate the effect of gestational nutrition guidance on the pregnancy outcomes of pregnant women with gestational diabetes mellitus(GDM).Methods From February 2019 to February 2021,80 patients with GDM treated in Foshan Maternal and Child Health Hospital were selected.They were 24-35 years old and 25-35 weeks pregnant,and their body mass index(BMI)was 22-28 kg/m2.They were divided into a study group and a control group by lottery,with 40 cases in each group.The study group took nutritional intervention during pregnancy,and the control group routine intervention.The blood glucose levels,nutritional intake levels,and pregnancy outcomes were compared between the two groups.Results There were no statistical differences in the levels of fasting blood glucose(FPG)and 2 h postprandial blood glucose(2hPBG)between the two groups before the intervention(both P>0.05).After the intervention,the levels of FPG and 2hPBG decreased in both groups;and the levels of FPG and 2hPBG in the study group were lower than those in the control group[(8.12±1.06)ng/ml vs.(9.53±1.75)ng/ml and(11.26±1.25)ng/ml vs.(13.23±1.58)ng/ml;both P<0.05].The study group had more intakes of vitamin A,vitamin E,iron,calcium,zinc,and protein and less intake of fat than the control group[(953.51±100.21)μg vs.(812.12±102.05)μg,(182.23±19.01)μg vs.(140.20±15.24)μg,(36.21±3.02)mg vs.(28.10±3.24)mg,(753.21±58.12)mg vs.(623.10±50.30)mg,(19.54±3.45)mg vs.(15.02±3.02)mg,(98.54±12.05)g vs.(80.15±10.12)g,and(80.21±8.12)g vs.(92.15±6.25)g;all P<0.05].The incidence of adverse pregnancy outcomes in the study group was lower than that in the control group[12.50%(5/40)vs.37.50%(15/40);P<0.05].Conclusion Gestational nutritional guidance during pregnancy has a good effect for women with GDM,effectively controls blood glucose,is conducive to nutritional supplement,and improves pregnancy outcomes.
