系统性红斑狼疮与牙周病间关联的Meta分析

摘　　要：目的采用Meta分析方法评价系统性红斑狼疮(systemic lupus erythematosus,SLE)与牙周病的相关性。方法全面检索相关文献,采用Stata12.0软件进行统计分析,共纳入13篇病例-对照研究文献。结果与对照组相比,SLE组牙周病风险明显升高[OR=2.55,95%CI(1.45,4.47),P=0.001],在牙周病的各个临床检查指标中SLE组探诊深度(PD)≥5 mm、临床附着丧失(CAL)、CAL≥2 mm风险明显升高[OR=2.09,95%CI(1.04,4.18),P=0.042];[OR=0.38,95%CI(0.15,0.61),P=0.001];[OR=4.37,95%CI(1.05,18.25),P=0.043],但两组口腔菌斑指数(PI)、探诊出血(BOP)、PD、龋失补牙数(DMFT)无明显差异。结论 SLE和牙周病之间存在显著关联,但需要更多的前瞻性研究来确认关联性。Objective To evaluate the correlation between systemic lupus erythematosus(SLE) and periodontal disease. Methods The relevant literatures were comprehensively searched and analyzed by Stata12.0 software. Results A total of 13 case-control studies were included. The results showed that compared with the control group, the risk of periodontal disease in the SLE group was significantly increased [OR=2.55, 95% CI(1.45, 4.47), P=0.001]. Among the various clinical examination items of periodontal disease, the risk of SLE group with depth of exploration(PD) ≥5 mm, clinical attachment loss(CAL) and CAL≥2 mm were significantly increased [OR=2.09,95%CI(1.04, 4.18), P=0.042], [OR= 0.38, 95%CI(0.15, 0.61), P=0.001], [OR=4.37, 95%CI(1.05,18.25), P= 0.043], but there was no significant difference in oral plaque index(PI), bleeding on probing(BOP), PD and the number of decayed-missing-filled teeth index(DMFT)between the two groups. Conclusion There is a significant association between SLE and periodontal disease, but more prospective studies are needed to confirm the relevance.
