摘　　要：目的系统评价CO_(2)点阵激光联合局部药物和(或)窄谱中波紫外线(NB-UVB)治疗肢端型白癜风的疗效。结果计算机检索Cochrane、Embase、PubMed、CBM、万方、中国知网、维普文献数据库,检索2005年1月—2020年8月在国内外期刊上发表的与治疗肢端型白癜风相关的随机对照试验(RCT)文献。提取数据运用Revman5.3软件以有效率作为疗效指标进行分析,计算比值比(OR)及95%可信区间(CI)。结果共检索到文献1224篇,根据排除标准共纳入7篇文献,经异质性、敏感性分析后剔除1篇,余6篇文献无异质性,采用固定效应模型进行总效应值合并。合并OR值3.1,95%CI(1.9,5.07),差异具有统计学意义(P<0.05)。结论CO_(2)点阵激光联合局部药物和(或)NB-UVB治疗肢端型白癜风的疗效优于单一和传统联合治疗(NB-UVB或梅花针联合局部用药),为肢端型白癜风的有效治疗提供新思路。Objective To systematically evaluate the efficacy of fractional carbon dioxide laser combined with topical medication and/or narrow-spectrum UVB in the treatment of acral vitiligo.Methods The databases of Cochrane,Embase,Pubmed,CBM,Wanfang,CNKI and VIP were searched by computer,and all RCT literatures related to treatment of acral vitiligo published at home and abroad from January 1,2005 to August 1,2020 were searched.The extracted data were analyzed by Revman5.3 software with the effective rate as the curative effect index,and the odds ratio(OR)and 95%confidence interval(CI)were calculated.Results A total of 1224 literatures were retrieved,and 7 literatures were included according to the inclusion and exclusion criteria.After heterogeneity analysis and sensitivity analysis,1 literature was eliminated,and the other 6 literatures had no heterogeneity.The fixed effect model was used to combine the total effect values.The combined OR value was 3.1,95%CI(1.9,5.07),and the difference was statistically significant(P<0.05).Conclusion The effect of fractional carbon dioxide laser combined with topical medication and/or narrow-spectrum UVB is superior to single and traditional combination therapy(narrow-spectrum UVB or plum-blossom needle combined with topical medication),which provides a new idea for effective treatment of acral vitiligo.
