内镜下套扎术与聚桂醇硬化注射治疗Ⅱ、Ⅲ期内痔的对比研究

摘　　要：目的:对比研究内镜下套扎术与聚桂醇硬化注射在Ⅱ、Ⅲ期内痔治疗中的效果。方法:以2019年12月至2020年12月于景德镇第二人民医院治疗的60例Ⅱ、Ⅲ期内痔患者为研究对象,数字表法分为注射组(n=29)和套扎组(n=31),注射组患者使用聚桂醇硬化注射治疗,套扎组使用内镜下套扎术治疗,比较两组患者围术期指标(手术时间、术中出血量)、恢复时间(住院时间、伤口愈合时间)。比较两组患者术后4 h、8 h、16 h时疼痛度[视觉模拟评分(VAS)]。比较两组患者治愈率和总有效率。比较两组患者术后1周内并发症发生率(肛门坠胀、出血、排便困难)。比较术后3个月两组患者复发率。结果:注射组患者手术时间、术中出血量、住院时间、伤口愈合时间均明显小于套扎组(P<0.05)。术后4 h、8 h、16 h时注射组患者VAS评分均明显小于套扎组(P<0.05)。注射组治愈率和总有效率均显著高于套扎组(P<0.05)。术后1周内注射组患者肛门坠胀、出血、排便困难不良反应发生率明显低于套扎组(P<0.05)。术后3个月,注射组复发率明显低于套扎组(P<0.05)。结论:聚桂醇硬化注射相较于内镜下套扎术治疗Ⅱ、Ⅲ期内痔患者,治疗时间和出血量均较小,且患者术后并发症发生率和复发率低,有利于患者预后。Objective:To compare the effect of endoscopic variceal ligation and lauromacrogol injection sclerotherapy in the treatment of stage Ⅱ or Ⅲ internal hemorrhoids.Methods:Totally 60 patients with stage Ⅱ or Ⅲ internal hemorrhoids who were treated in Jingdezhen Second People’s Hospital from December 2019 to December 2020 were selected as the research subjects,and they were divided into injection group (n=29) and band ligation group (n=31) according to the digital table method.Patients in injection group were treated with lauromacrogol injection sclerotherapy,and patients in band ligation group were given endoscopic variceal ligation.The perioperative indexes (surgical time,intraoperative blood loss) and recovery time (hospital stay,wound healing time) were compared between the two groups.The pain level[Visual Analogue Scale (VAS)]of the two groups was compared at 4 h,8 h and 16 h after surgery.The cure rate and total effective rate were compared between the two groups,and the incidence rates of complications (anal bulging,bleeding,difficulty in defecation) within 1 week after surgery were compared between the two groups.The recurrence rate of the two groups was compared at 3 months after surgery.Results:The surgical time,intraoperative blood loss,hospital stay and wound healing time of injection group were significantly shorter than those of band ligation group (P<0.05).The VAS scores of patients in injection group were significantly lower than those in band ligation group at 4 h,8 h and 16 h after surgery (P<0.05).The cure rate and total effective rate of injection group were significantly higher than those of band ligation group (P<0.05).The incidence rates of adverse reactions such as anal bulging,bleeding and difficulty in defecation in injection group were significantly lower than those in band ligation group within 1 week after surgery (P<0.05).At 3 months after surgery,the recurrence rate of injection group was significantly lower than that of band ligation group (P<0.05).Conclusion:Compared with end
